During the inspection it was found the diaper changing mat used to change the infant diapers was also being used as a nap mat. This was corrected at the inspection when the caregiver removed the mat and replaced it with a nap time cot.

During the follow up inspection, there were 8 children total at the childcare home. The childcare home is licensed for a total of 6 children.

During the follow up inspection, two family members were observed in the home without an active background check.

During the follow up inspection the safety plan was observed to not be followed for the ratio and background checks.

The primary caregivers cpr and first aid training is expired. The training expired in February 2026.

During the inspection, there were 8 children total at the childcare home. The childcare home is licensed for a total of 6 children. This was corrected at the inspection when the primary caregiver sent 2 of the children home.

During the inspection, the restroom the children have access to was observed with dirt and rust in the toilet. Peeling paint was observed on the door frame in the room the children were located in.

During the inspection, a family member was observed in the home without an active background check. This was corrected at the inspection when the person left the home during the inspection.

There was no fire drill, lock down drill, severe weather drill, or sheltering drills conducted for the current calendar year.

One of the children files reviewed did not have a health statement.

There was no soap or towels available in the bathroom area. This was corrected at the inspection when the caregiver placed soap and paper towels in the bathroom area.

A trampoline was observed in the back yard and accessible to children.

Trash and two lawn mowers were observed in the back yard play area where children have access to.

Standing water was observed in the water play table. This was corrected at the inspection when the caregiver emptied the standing water.

The operational policies were missing the parent rights, emergency preparedness plan, and discipline policy.

Two children under the age of two were observed having screen time. This was corrected at the inspection when the caregiver turned the screen off.

The primary caregivers CPR and first aid is expired.

During the inspection the fire extinguisher was not mounted.

During the inspection the first aide kit was observed but did not contain tweezers, scissors, or a thermometer.

During the monitoring inspection there was one adult present in the home, who is a household member, and they did not have a background check active in the system. Household member's background check was entered while I was present in the home.

During the inspection children's records were reviewed and both files reviewed were missing admission information such as medical authorization, emergency contacts, and health statements.

During the investigation it was observed that the children were not following the written activity plan that was posted.

During the investigation inspection it was observed that alcohol bottles, empty cans, and an ashtray were in the backyard, within reach of the children in care.

During the annual monitoring inspection it was found that the fire extinguisher was being kept under the sink and should have been mounted.
