The children did not wash their hands after being assisted to the toilet.

The diaper changing mat was observed with a hole in it and was not sanitized by the caregiver after being used.

The caregiver did not wash their hands after assisting the children with toileting.

Caregivers were not supporting the children in a positive way. The caregivers displayed frustration with some of the behaviors the children were demonstrating and were observed and heard reacting negatively with the children.

Caregivers were observed moving children around and handling them in a frustrating-aggressive manner. A caregiver was also heard yelling and humiliating a child while the inspector was in the operation conducting an inspection.

Caregivers did not have current CPR and First Aid.

A broken bicycle was observed on the playground where a group of children were playing, posing a risk of injury to the children. This was corrected during the inspection when the bicycle was removed from the playground.

Screentime activities were observed in a classroom of children under two years of age. This was corrected during the inspection, when the caregiver turn the screen off.

4 of the required 8 drills had been completed. 4 severe weather/shelter in place drills and 4 lock down drills must be completed annually for a total of 8 drills.

Components of the written operational policies were missing from the parent handbook. Parent's rights, emergency preparedness plan, suspension.

During the inspection, numerous toys and equipment was observed torn, tattered and broken. The items cause pinching and other hazards that pose risk to children.

During the inspection two ceilings fans located in classrooms used by children in care were found to be covered in dust. This was corrected at inspection when the fans were cleaned. During the inspection a caregiver's purse was observed to be on a shelf in an area where children were playing. This was corrected at inspection when the purse was removed and stored in an area that is out of reach of the children in care.

Children's potty chairs were in a classroom with waste inside of them. This was corrected at inspection when the potty chairs were removed from the classroom. Cleaning chemicals were observed on a bookcase within reach of the children in care. This was corrected at inspection when the items were removed and stored out of reach of the children in care. Garbage cans in two classrooms were found to have no lids and dirty diapers were on top within reach. The fence surrounding the playground areas were found to be rolling up and presenting an entrapment hazard.

A transportation vehicle purchased less than 3 years ago was found to not have an electronic child safety alarm.

An operation staff member stated they place children in time out for 5-10 minutes, which exceeds the required minute per age for their age group. This was corrected at inspection when appropriate time frames were discussed with the staff member.

During the inspection two separate classrooms were observed to not wash the children's hands after changing their diapers.

Activity plans observed in the operation's classrooms did not include outside time for any of the age groups and were not detailed as to the activities the children engage in.

3 of 7 children's files reviewed were found to not have an admission date. 2 of 7 children's files reviewed were found to not have the field trip admission information completed. 2 of 7 children's files reviewed were found to not have water activity permission completed. 2 of 7 children's files reviewed were found to not have health care information completed. 2 of 7 children's files reviewed were found to not have a health statement on file.

A fire extinguisher found in a transportation vehicle was found to have the needle on the undercharged section. This was corrected at inspection when a new fire extinguisher was placed in the transportation vehicle.

The nap room was made up of mixed ages, specified age group for the room was 22 making the room out of ratio by 11. The infant room was found to have 10 children in care with one caregiver, making the room out of ratio by 6. This was corrected at inspection when an additional caregiver came into the room.

The first aid kits reviewed in the transportation vehicle was found to have an ice pack that exploded inside, some items were missing, and had expired medication. This was corrected at inspection when a new first aid kit was placed in each transportation vehicle which has all required items in them.

Playground equipment at the operation was found to have two slides that do not have the required six feet use zone.

A caregiver, responsible for a group of children in care, was found to be asleep.
