During a walk through of the playground, I observed a portion of the back gate that has damaged and no longer attached. There were boards with nails lying on the ground and accessible to the children. The back porch had electrical cords and lawn equipment out and accessible to the children. A safety plan was developed with the provider stating how the operation would be able to promote outdoors play while keeping the children safe outside. I also observed repairs to the gate being done.
