A caregiver did not wash the hand of a child after diapering. This was corrected when the caregiver returned and washed the child's hands.

A caregiver was observed transitioning a group of children to the bathroom. A child in the care of the caregiver kept leaving the room after being told not to. The caregiver was also observed allowing a group of children to return to the classroom alone while two other children remained in the bathroom. The children returned back to the group when finished.

Staff stated a child was placed in a highchair, to keep the child from hitting other children.

Staff and caregivers did not use good judgment when interacting and discipling a child in care.

The walls in 3 of the classrooms and in the dining room had peeling paint and needed to be wiped down. Trash cans did not lids and needed to be wiped down.

Children's records was missing parent signatures, and admission dates.

One operation staff did not have the required pre-application affidavit.

The emergency alarm in one of the vehicles used for transportation was dislodged.

A caregiver was observed in a room with 5 children, bringing the room out of ratio by 1 child. This was corrected when another caregiver entered the room.

A caregiver opened the door for CCR leaving a group of children unsupervised in the classroom. The caregiver returned to the room with the children.

A caregiver was observed not being in a vicinity of a child that threw a chair and hit another child in care.

Two vans used for transport was observed and did not have the required safety alarm installed.

Bathrooms were observed with no tissue or paper towels for use by the children. A bathroom was observed with a trash can with no lid. The can had left over garbage inside. A toilet was observed with urine and fecal stains inside and out. Mops and mop buckets were hanging over a mop sink in the bathroom and accessible to the children. This was corrected during the inspection. The provider cleaned the toilets and removed the garbage.

During the inspection, play yards were observed with matresses that did not fit flat against the sides.

Caregivers were observed to not be washing their hands after arriving at the operation.

There was no loose fill installed under slide exits and rocking equipment.

From interviews and information gathers from the investigation it was determined that the operation was utilizing prohibited punishment.

During the inspection it was found that the operation did not have written feeding instructions.
