A copy of the most recent fire inspection was unavailable during the inspection. The last inspection form was dated 10/15/2024. NOTE: The Fire Marshal was contacted during the inspection visit, verified they had been out for an inspection but could not locate the report. Another inspection was scheduled for Monday.

The operation's last gas inspection was conducted on April 9, 2024 and a new gas inspection had not been conducted. NOTE: A gas inspection was scheduled for tomorrow during the inspection.

Four caregivers did not have a notarized affidavit on file.

Two caregivers were missing documentation of completing pre-service training and three caregivers did not have signature of the trainer who completed the orientation.

One caregiver completed 16 out of 24 annual hours for the evaluation year 7/2025-6/2026.

It was observed that a member of the management team physically pulled a child by the arm toward and inside the classroom while the child physically resisted.

One classroom observed had the classroom toys kept on top of the cubbies, inaccessible to the children.

Five out of eleven staff files evaluated had not completed the required 24 annual training hours for their evaluated periods.

Children have been served burnt biscuits, uncooked oatmeal and bananas that contained growing mold on the stem. Milk is being stored in classroom mini refrigerators resulting in the milk freezing. Frozen fruit is not being fully thawed before serving to children.

Four infants, ages 2-9 mos of age did not have a current feeding instruction form on file.

One out of ten staff files evaluated had completed 20 out of 24 annual hours.

Three out of ten child files evaluated did not have updated immunization records on file.

A caregiver and child had similar store-bought water bottles in the classroom. Due to the water bottles not being labeled properly, a caregiver's water was poured into a child's drink cup.

1 infant bottle and two sippy cups were not labeled with the child's first name and last initial. NOTE: This is a re-cite.

An infant caregiver was observed not changing gloves in between diaper changes and not washing their hands after diapering. The caregiver used hand sanitizer only.

A classroom initially had 2 caregivers with children ranging in age 2-3yrs, but when the second caregiver left the room, the specified age group was 2 yrs leaving the room out of ratio.

Two out of four bottles observed on the counter were not labeled with the child's name for identification.

Two infant children were observed not having their hands washed after being diapered.

A school-age child was observed in the staff lounge that is adjacent to the kitchen instead of a classroom. NOTE: The child was removed from the space and went home during the inspection.

One out of ten child files evaluated was missing updated vaccination records.

A 15 month old child was left without supervision on the playground.

Four infant feeding plans evaluated for children ages 5 mos, 5 mos, 10 mos, & 10 mos did not have a current plan on file.

A box of baking soda was accessible to children in the bathroom.

The operation completed an incident report 5 days after the incident occurred.

Two 3 year old children exited the playground and crossed 4 door thresholds resulting in exiting the front of the building into the parking lot without an adult.

Parent signatures were not obtained for the incident reports completed.

An infant caregiver did not wash their hands after diapering a child.

1 out of 10 child files evaluated was missing a current vaccination record. The child is 3 mons of age and is missing their DTaP, IPV and PCV vaccinations.

3 out of 10 staff files reviewed did not have a CPR certificate on file.

The last recorded fire drill was conducted on 7/21/22.

5 children were observed sitting in bucket seats to eat breakfast in the infant room and the seat belts were not secured around the children. NOTE: This was corrected when the caregiver secured the children in each seat using the available safety straps.

3 out of 10 staff did not have a First Aid certificate on file.

The director was made aware by a parent that there was an allegation a child was potentially placed at risk and did not notify licensing of the concern.

The operation did not report to licensing within 48 hours that a child who was in care obtained medical attention. An Administrative Penalty will be recommend as a result of this citation, per HRC 42.078.

Once child with a food allergy plan on file did not have the original prescription box on file for their prescribed medication.
