There is a broken window in one of the prek classrooms that is covered by cardboard. There is broken glass still in the window frame. There are also numerous trashbags throughout the center and infant playgrounds that contain old or broken toys and children were observed playing around and under the bags.

A classroom with a specified age group of 2 yr olds had 23 children with two caregivers. The class was over by one child.

A classroom measured for 10 children had 23 children ages 13 mos-3 yrs of age in one classroom.

There were 5 children ages 13 mos, 14 mos, 16 mos and two 17 mos olds combined with 3 year old children. There was a 23 month age gap between the youngest and oldest child.

A school-age child was observed exiting one building, walkthing through the breezway, and entering the main building and their classroom alone.

The operation last had a fire inspection in April 2025.

Two children with medication kept at the operation for allergy plans were expired. One expired January 2026 and the second May 2026.

A 7 month old infant was observed being placed on their stomachin their crib to sleep.

An infant was observed in their crib wearing a bib. The infant was rolling from back to stomach.

A caregiver was observed not washing their hands after changing a child and before beginning to change a second child's diaper.

A child's hands were not washed after being diapered and was placed to sleep in their crib directly after the diaper change.

A caregiver was observed putting gloves on to serve lunch and did not wash their hands.

At the time of inspection the caregiver in the Kinder Ready room did not know how many children were in her care. She stated there were 20 children in their care when there were 21.

A caregiver was overheard telling children to stop, get on red, that's what happens, should have been listening, and go sit down to correct the behaviors of the children in the class.

A caregiver failed to conduct a name-to-face attendance check when bringing in the classroom from the playground to the classroom resulting in a child being left on the playground for a minute and a half.

The metal slide on the Kinder Ready playground exceeds 5 ft from the use zone surface below to the platform of the slide. Children that utilize the playground include 4 yr olds.

A Health Inspection was not available for review during the inspection.

A toilet in Toddler 2 was being used as storage and not accessible to children.

There were 4 stationary toys on the preschool playground that were under 6 feet from another piece of equipment.
