A water hose attached to the faucet off the infant back door had not been secured.

There were two out of ten child files evaluated missing their enrollment start date.

Seven out of 10 child files evaluated were missing or had incomplete emergency contact addresses.

The loose fill around the two playscapes and under the swing set is less than an inch in depth or not evident at the end of the slides.

One out of ten child files evaluated was missing a health statement upon enrollment in October.

There is a protruding circular piece of metal in the rock area of the older playground and a covered water pipe full of water in a portion of the playground where children have to pass through to get to the back playground. Children can lift the lid and have access to the standing water.

Two out of ten child files evaluated did not contain a copy of the child's vaccination records.

During the investigation, it was found that a caregiver who had unsupervised access to children did not have first aid certification.

A caregiver used poor judgment when they pushed an infant's head onto a mat they were laying on outside and began rubbing the side of the infant's head on the mat in order to clean their face after the infant spit up. The same caregiver was later observed picking up a child by one arm and carrying them upside down to return them to their cot during naptime. A second caregiver was observed carrying a child right below their wrists to another caregiver while on the playground.

A caregiver was observed pulling a child by the leg twice while they were sitting outside to bring the child closer to the caregiver. The second time, the child fell backwards, hitting their head on the ground. The same caregiver was observed pulling a child by their clothing and pushing them away from an object while on the playground.

During the investigation, a caregiver reported putting a book on a child's back during naptime to create the feeling of a weight to put the child to sleep.

The last Health Inspection was conducted on 11/6/2024.

Three classroom restrooms had mop buckets and water accessible to the children. Classrooms use soap and water buckets to clean table tops. Spray bottles were accessible in the restroom to children in the prek room. There was a toilet brush laying on top of the toilet lid in the 2 yr room.

On the older playground, there was a balance beam and a bench within 3 ft of eachother.

There wasn't a health statement on file for the pet guinea pig in the Kinder-ready room. NOTE: The caregiver took the guinea pig home during the inspection.

There wre three metal, stationary riding toys anchored to the ground that had the metal base exposed.

The red climber on the 2 yr old playground had peeling paint. One blue boat on the prek playground had broken boards. There were two carpets in the school-age room that were curling on the edges.

Two trash cans in the prek room restrooms, one in the toddler room restroom, and one outside of the gym restrooms did not have a lid.

A caregiver relieved another caregiver in the infant room for break and did not have an eligibility letter on file. NOTE: The caregiver was sent home and a background check request was entered during the inspection.
