Based on the information obtained from the DFPS investigation, the bus driver failed to properly supervise the child in care. The bus driver did not use good judgment by allowing the child to remain asleep on the van when the child stated he did not feel well. The child should have been brought in the building, and the mother should have been called to inform her of his condition.

The loose-fill surfacing under rotating playground equipment, slide, and merry- go- round was less than six inches deep.

Director denied inspector access to interview children, this access was delayed by operation staff on 09/02/2025. Director failed to provide the daily list of the children being transported from 08/25/2025 to 08/29/2025 for review during the investigation.

Outdoor play equipment was moved. The use zone for the outdoor space geometric dome climber and rotating playing equipment overlapped.

The multi-color play structure with slides was placed on grass. It did not have surfacing.

The operation did not submit the number of employees who left employment by the due date and extended due date due to portal issues.

The naptime child/caregiver ratio was not met as two divided classrooms were being supervised by one caregiver. The caregiver was sitting in the middle of two classrooms. One classroom had 11- 4-year-old children napping. The other classroom had 10 children napping with a specified age group (SAG) of 3 years old children napping; 7 of which were 3 years old.

Outdoor play equipment was moved. The use zone overlapped with other stationary and rotating equipment, the school bus, outdoor space geometric dome climber, and rotating play equipment.

The slides' perimeter is obstructed by the barriers used around the play area of the slides. The use zone of the slides is less than 6 feet for the green and blue slides.

The play equipment lacked the required surfacing in the use zones.

One child's record was due for the health statement from a health care professional.

The required annual training on the following topics had not been completed by caregivers. The required training topics are 2. communicable diseases to include immunizations 4.preventing and responding to emergencies due to food or an allergic reaction and 6. handling, storing, and disposing of hazardous materials.

One child's record lacked the updated immunization on file.

There was a 7 month old infant with a mix group with the oldest children being 4 years old.

On 09/13/21, a staff member counted in child/caregiver ratio of a group of children with another staff member attended to other administrative duties by stepping away from the classroom to open the door or answer the telephone.

The parents have not been allowed to enter the building after the emergency rule expired on 04/20/21.
