The required annual training on the following topics were not available for review on 2 caregivers. The required training topics are 3. administering medication 4.preventing and responding to emergencies due to food or an allergic reaction and 6. handling, storing, and disposing of hazardous materials.

Two children's records indicated by the parent their child had a food allergy. Ther was no emergency care plan on file.

The required annual training on the following topics were not available for reveiw for the director; 6. handling, storing, and disposing of hazardous materials.

One child's record lacked the updated immunization record for the child's age.

Three staff did not have proof of education in the file for review.

Two children enrolled in care did not have the required physician health statement.

One child enrolled in care did not have authorization for emergency medical care and consent to transport.

In the schoolers classroom, 2 out of 12 children were observed sleeping, 5 were observed on electronic devices and the other children were observed watching a movie during naptime/rest time.

A caregiver failed to demonstrate the use of good judgment when she threatened to call law enforcement on a small child who would not listen.

Two caregivers were observed laying down in the dark during naptime and not supervising children.

Multiple children were observed engaged in screen time during nap/rest time. Some children were watching television, some were on tablets and one child was observed on a cell phone device.

Medication for a child in care was not in the original container labeled with the child s full name and the date it was brought into the operation.

The food allergy emergency plan form was not signed and dated by the parent.

The operation did not have medical treatment authorization forms for each child in being transported.

Based on the preponderance of evidence there is sufficient evidence to prove that supervision of a child in care was not being adequately maintained. The child has been observed by staff and parent(s) outside of the classroom (roaming the building) without adult supervision.

An 11 month old infant was observed sleeping on the boucer during the investigation inspection walkthough. The infant was removed from the bouncer and was placed in the crib for napping

Based on the information obtained from the investigation, it was confirmed that children were left unsupervised when a caregiver had to leave their classroom to retrieve a child from running outside of the classroom.

The director had completed 15 hours of 30 hours of annual training (August 2020-August 2021) at the time of inspection.

A restroom used by children in care had diaper changing supplies (Caldesene Baby poweder, Honest soothing bottom wash, 2 Boogie Bottom Diaper Rash Spray) that were within reach of children.

A 3 month old infant was swaddled in a crib while sleeping. The swaddle was removed at the time of inspection.

The director failed to ensure that training requirements and safe sleep practices were being met at the operation.

Two caregivers supervising infants at the time of inspection did not have CPR/First aid training. A caregiver supervising infants did not have current CPR or First Aid training. The certification expired in May of 2021.

An employee hired on or around July 2019 did not have a signed/dated proof of orientation and preservice at the time of inspection.

An infant caregiver did not have annual training in the following topics: Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS) and Understanding early childhood brain development.

An infant was observed sleeping with a pacifier attachment with a clip that was attached to the infant. This was removed at the time of inspection.

Emergency medical authorization information was not located in the vehicle used to transport children.

A group of children under the age of 2 years old were observed watching Baby Shark at the time of inspection.
