Two children's records lacked the immunizations for their respective age on file.

The breakfast was being prepared on the hand washing sink. This was corrected at the inspection when the inspector informed the director and caregivers.

The staff (cooks) are being used to meet child/caregiver ratio in the classrooms.

There were 5 infants in infant 1 classroom with 1 caregiver. The specified age group (SAG) was 11 months.

There was a staff (cook) who was substituted to open toddler 1 classroom who lacked the minimum requirements for a caregiver.

The admission forms used were outdated. The admission forms do not capture the required minimum standards changes of March 2023.

One of the van, the last row of the van lacked the latching mechanism. This was corrected at the inspection. A new seat belt was placed.

A child's record indicated a food allergy. Although a medical note was on file identifying the allergies, there was no emergency care/food allergy emergency plan on file.

Two children's record did not have the school information on the admission form.

A classroom, prek 2, had 17 children with one caregiver. The specified age group (SAG0 was 3 years old. The classroom was over ratio by 2 children.

Lock down drills were not conducted in the 2023.

Two caregivers records lacked the educational documentation on file.

One child's record lacked an immunization record on file. Three children's records lacked an updated immunization record.

Six infants in infant 1 did not have the written feeding instructions sheets reviewed and updated by the parents within the last 30 days. The most current one was last reviewed and signed in January.

In the toddler playground, the 2 play structures measured less than the required 6 feet of use zone. The use zone overlapped. This was corrected at the inspection when the equipment was moved.

This was last validated on 11/16/2022. This was corrected at the inspection.

The wooden play structure had different areas of rotten wood.

The stationary equipment was moved; however, it did not meet the requirement as the use zone overlapped with other stationary equipment. The stationary equipment was moved during the inspection by the owner.

There were 3 stationary equipment which did not meet the use zone measurement.

There were 7 infants with one caregiver. The specified age group (SAG) was 15 months. With SAG of 15 months, the child/caregiver ratio was not met.

Three caregivers in the classroom did not have an active background check status.

The caregiver was using the operation's electronic device to submit information after the children completed having a snack. The children were engaged in free play.

The child/caregiver ratio was not met for the toddler classroom. The caregiver had 4 children on a buggy and 3 children in the classroom. The children had returned to the classroom from the playground. The specified age group (SAG) was 15 months. The child/caregiver ratio was over by 2 children in care.
