The Franchise Tax account was showing as inactive in the system during the inspection.

In the infant's classroom, there were two children (3 years old) and one infant (15 months old) combined during inspection time. Note: The director removed the two children (3 years old) to the toddlers' classroom during inspection.

During the inspection, an individual who works directly with children did not have an active initial background check under the operation number.

The playhouse has a stair with broken wooden pieces, and the snail is stuck out with those broken pieces at the playground.

The fire marshal's inspection report was not available for review. The current report expired on 11/09/2024.

There were 2 caregivers who were present and have been in direct care of the children prior to today. They did not have the background check with status eligibility.Their BGC are still pending. Note : The director sent them home and called another caregiver to come in.
