A shop vac, a board with screws, and a trash bin with an old carpet and broken table leg was left in the restroom of the afterschool building. In addition, the toilet was missing the tank lid. This was corrected at the inspection when it was addressed and all items were removed from the restroom. The tank lid was also placed back onto the toilet during the inspection.

A spray bottle of soap and water used for cleaning was left accessible to children in care. Cleaning spray bottles were also stored near/above food items. This was corrected at inspection when it was addressed and the cleaning supplies were picked up and made inaccessible to children in care. Cleaning items were also moved and stored away from food.

Three out of ten child records evaluated were lacking a health statement signed by a medical professional.

One out of eight employee records evaluated were lacking one hour of training in child maltreatment for the most recent completed training year.

Three out of ten children records evaluated were lacking up to date immunization records on file.

Two employees did not have training in pediatric first aid prior to having unsupervised access to children in the classroom.

Two caregivers did not have training in pediatric CPR prior to being unsupervised with children in the classroom. They were the only caregiver present in their respective buildings.

Children were redirected inappropriately by two different caregivers during at least two separate incidents. A child was also left alone on the playground for a few seconds before another caregiver stepped in. The operation did not notify the parents of any of these incidents yet felt strongly enough to reprimand or address the issues with the caregivers. This demonstrates a pattern for a lack of good judgement.

In two separate incidents, two different caregivers threatened corporal punishment to redirect children.

A caregiver left a child unsupervised when they exited the playground with the rest of the class and left a child behind for a few seconds. The gate to the playground was latched closed. There were no other caregivers present on the playground with the child.

Three out of five classrooms had a screen with a moving image during naptime. This was corrected at inspection when the screens were turned off.

Based on the preponderance of evidence gathered during this investigation, a caregiver exercised poor judgement when they showed a video of an upset child in care to an individual not associated with the daycare on their personal device.

A child's hands were not washed after having their diaper changed. This was corrected at inspection when it was addressed with the caregiver and the child's hands were washed.

There was a floor tile that was broken and lifting up in an infant room. An infant had a piece of tile in their mouth.

A caregiver did not wash their hands after changing a child's diaper. This was corrected at inspection when it was addressed with the caregiver and the caregiver washed their hands.

School-age children did not wash their hands prior to sitting down for snack. This was corrected at inspection when it was addressed with the caregiver and the children washed their hands.

4 outlets in the pre-k room were not covered. This was corrected at inspection when outlet safety covers were placed over the outlets.

The diaper changing area was not sanitized in between diaper changes. This was corrected at inspection when it was addressed with the caregiver and the diaper changing area was then sanitized.

A 4 year old child was seperated from the group for time out for approximately 30 minutes.

A 4 year old child was asked during time out to stand still on a tape on the floor for about 4 minutes to approximately 30 minutes.

A caregiver had a 4 year old child in the kitchen while they were cooking during time out. The kitchen contains chemicals, sharp objects and other items that must be kept out of reach of children.

A dog was brought into the center and they allowed the children around this dog without notifying the parents in writing.

The director allowed a child to be put in time out in the kitchen, knowing it violated minimum standards.

One out of five infants was observed falling asleep while drinking a bottle in the crib.

Two out of five infants had a bottle proped with a blanket.

One infant was observed sleeping with a pacifier with a stuffed animal attached to it. The caregiver corrected the deficiency by removing the pacifier with the stuffed animal.

The two year old caregiver was observed yelling at the children.

Multiple infants were observed to be sleeping in the cribs with blankets covering them.

One 8 month old was observed to be sleeping in a restrictive device.

Recited: Two bottles in the infant room was not labeled the child's name.

The two year old caregiver was observed watching a video on a laptop while using headphones during naptime.

Two infants were observed to be swaddled while sleeping without the sleep exemption form.

Lighting was inadequate in the three year old classroom during naptime.

One eight month old infant was observed with a bottle in the crib.

Two bottles in the infant room was not labeled the child's name.

A caregiver was observed wiping a child's nose and not washing her hands afterwards.

One infant was sleeping in a crib with yarn tied around the child's neck holding a pacifier.

The play scape lacked four inches of the required six inches of loose fill.
