10 children's records were reviewed. Two kids are missing current immunization records from their file.

The loose fill of mulch measured 2 inches in the use zone for all play equipment requiring 6 inches.

The center's director is missing annual training in required topics: SIDS/ Shaken Baby Syndrome/ Early Child Brain Development, Emergency Preparedness, Communicable Disease, Administering Medication, Food Allergy, Building Safety, and Hazardous Materials.

An 10 month old infant was sleeping in the crib with a bib around the neck. Note: this was corrected at inspection when the caregiver removed the bib from around the neck.
