Kids' Zone Downtown

A two year old was able to get beads that are a choking hazard for toddlers. This has been corrected as the toddlers will no longer combine in any of the older childrens classrooms that have small objects such as beads.

During an investigation it was found that a child had an accident during naptime and was in the soiled clothes until 5:30 when the child was picked up. The child was in the soiled clothes for around 3 hours. I spoke with the director about ensuring that children are going to the bathroom before naptime and checked after nap is over.

A child was able to find medication inside the operation and had the medication in their hand.

Observed a broken wagon outside where children were playing. The plastic had sharp edges that could hurt a child. This was corrected whent he toy was removed from the playground.

The operation was due for a sanitation inspection in 4/2026 and it had not been scheduled and was past due.

Observed 3 infants feeding instructions that were completed in February and March and had not been updated monthly. I spoke with the caregivers about updating the feeding instructions monthly.

Observed 10 children records, three children needed updated immunization records. This was discussed with the director.

Obsserved mutiple infants sitting in the table chairs without being strapped to the seat with the seat belts.

During a walk through of the building, one classroom stated that they had 12 children but actually had 13 children. When licensing asked again how many children they had, they stated they had 12 children. Licening had the caregivers count the children again and then they realized that they had 13 children in care and one hadn't been signed in for the day.

Observed 8 infants sitting in the highchair table and were not strapped into the chair. This was corrected when the infants were removed from the seats.

Observed an infant sleeping in a crib on top of a boppy pillow. This was corrected when the boppy pillow was reemoved from the crib.

The operation had not completed a fire drill in July 2025. This was corrected when the fire drill was completed during inspection and the children and staff were able to exit in under 3 minutes.

Observed one classroom that had two classrooms mixed together due to low numbers. The two caregivers did not know how many children they had in care. They stated that there was 17 children and one caregiver had 9 and the other had 8 children but there was only 16 children in care and each classroom had 8 children total.

The operation's current health inspection is out of date and was last completed January 2023.

There was sufficient evidence found during the investigation to show that children were sent to the bathroom without direct supervision.

A fire inspection has not been completed within the last year.

Observed one infant room with 5 infants 4 to 8 months old with one caregiver. This was corrected when a caregiver came into the classroom. I observed a second classroom with 6 infants ages 12months to 16 months old with one caregiver. This was corrected when another caregiver came into the classroom.

A caregiver placed an infant face down on a boppy pillow for tummy time and walked away from the infant. While the caregiver was away from the infant, the infant scooted down in the pillow and was unable to get out of the pillow themself.

Observed multiple foam and plastic cups that were scattered over the large playground area. I spoke to the oepration about ensuring the playground is kept clean.

There was a 7 year old child in the same classroom as infants who are 13-17 months old. This was corrected when the 7 year old was taken out of the room.
