The operation failed to abide by the 18 months rule difference when a mixed age group classroom was observed with 15 children including a 5 months old and 6 years old combined.

A 4-month-old infant was observed sleeping with a pacifier clipped to their chest during the inspection. This was corrected when the caregiver removed the clip attachment from the pacifier.

The television in the Pre-K/ School Age classroom was observed being left on during rest time. This was corrected when the caregiver was made aware and turned off the television.

Sleeping space in the Pre-k/ School-age classroom was observed not adequately arranged when there were no spaces observed between the children's nap cots. This was corrected when the napping area was rearranged.

Supervision was not maintained when a caregiver in the 2-year-old classroom was observed standing in the hallway outside of the classroom and not supervising children during nap time.

The television was observed being left on for over one hour during the inspection. The caregiver in charge was made aware and turned off the TV during the inspection.

Two employees and the director's files were reviewed and observed missing the proof of high school or equivalence documentation in their personnel files.

The food allergy plan for one child age 2yo was observed missing from the admission information forms.

Based on the information obtained from the investigation, although the operational policies stated parents are welcome to visit the operation at any times, the operation is implementing closed door policies with no wirtten explanation to the parents who have their child enrolled at this operation.

The operational polciies was reviewed and observed missing the information related to the duration for physical indoor/outfoor activities for the children in care.

The annual training for one caregiver was observed missing 17 hours of the annual training period 11/20-11/21.

Supervision for children was not maintained when there were no teacher in the Toddlers Classroom with 7 children ages 2-3yo was observed present, two of the children were observed napping when CCR inspector entered the classroom.

The liabilty insurance for the child care center was obseved expired on 3/30/2022.

The gate to cover the electrical AC units in the outdoor area was observed left open. The smaller playground was observed having puddles of water standing on concrete surface where children are accessible to This was corrected when the gate for the AC units were closed and locked, the director removed the standing water in the playground area.

The monthly playground inspection was not conducted for the playground at the operation

The daily playground check list was observed not completed for the current date 3/31/2022. This was corrected, the person in charge completed the playground check list and documented in the form.
