Of the 10 children's records evaluated, 5 lacked the address of the emergency contact.

Of the 10 personnel records evaluated, 2 lacked training in emergency preparedness, food allergies, administrating medications, understanding building and premises safety and handling and storing of hazardous materials.

Safety drills for lockdown procedures are not being conducted.

Original training certificates were not available for review. Each file must contain original training certificates for pre-service and annual training hours.

A 4 month old child was observed sleeping in a restrictive device. The child had been asleep for 30 minutes.

This standard was evaluated in regard to a report received and compliance was not met. A child in care was released to a person not listed on the authorized persons to pick up the child.

Six of the staff folders reviewed was observed as not having the required Handling, Storing, and Disposing of Hazardous Materials training.

The operational policy was observed missing the required information on promotion of indoor/outdoor physical activity.

Six of the staff folders reviewed were observed not having the required training for preventing and responding to emergencies due to food and allergic reaction.

Based on the information obtained during the investigation, there is sufficient evidence to support caregivers did not wash their hands after diapering children.

Based on the information obtained during the investigation, there is sufficient evidence to support two caregivers were using a cell phone while supervising children during naptime.

Based on the information obtained during the investigation, there is sufficient evidence to support that a caregiver left a child unattended on a changing table not keeping their hands on the child while child was on the changing table.

Based on the information obtained during the investigation, there is sufficient evidence to support that caregivers did not wash children hands after diaper changing.

Based on the information obtained during the investigation, there is sufficient evidence to support that a caregiver did not demonstrate good judgement when the caregiver was seen on video flipping a child off a cot onto the floor when waking the child up from naptime, and grabbing another child by the arm and ankle lifting the child from the cot and placing the child on the floor while awaking the child from naptime. The caregiver did not demonstrate good judgement when she was viewed on video leading a child by the hand and slinging the child down roughly where she wanted the child to sit. There was not enough evidence to support that the caregiver was roughly wiping a child during diaper changing.

None of the infants in the infant room have completed infant care sheets signed by the parents.
