Based on information obtained during the investigation, a caregiver admitted to tapping a child on the hand as a form of discipline.

During the inspection the bathrooms were observed to be unsanitized. There was dried pee and dirt on the bottoms of all 6 toilets, this was re-cited.

During the inspection there was a individual present, with access to the children, who did not have a completed background check.

During the inspection it was observed that the children in the pre k classroom were sat down at the table and were beginning to be served lunch without having washed their hands first.

During the inspection it was observed that one of the first aid kit backpacks was hanging on a low shelf with the children's backpacks, within their reach.

While reviewing files it was found that at least 4 children files did not have emergency medical authorization.

During the inspection the bathrooms were observed to be unsanitized. There was dried pee and dirt on the bottoms of at least 4 toilets, the trash can lids and outside of the trash can were visibly dirty, and the at least 3 of the sinks appeared to be dirty with visible dirt and residue.

During the inspection it was observed that the operation did not have any documented lock down drills for the year 2024.

During follow-up inspection today, I observe closet in the toddler room unlock which contained various hazardous items inside. Note: corrected during inspection.

Two staff present missing current first aid training.

Four caregiver's records evaluated were missing one hour of child maltreatment training.

Two staff present missing current CPR training.

Four personnel records were missing documentation to show hire date, required training completed.

I did not observe fire drills and carbon monoxide detector tested and documented for November and December 2023.

One staff record was missing notarized affidavit on file.

The toilet in Pre-K room was not operating and unavailable for children to use. I observe several vehicle tires in playground retaining water inside presenting a safety hazard.

The daily activity plan not posted in toddler room.

I did not observe current background check completed for caregiver present. Note: corrected during inspection.

The updated fire inspection report has not been submitted to Child Care Regulation verifying that the operation has the inspection.

The most recent fire inspection was conducted on 3/3/2022. There is no documentation that an annual fire inspection was completed.

2 out of 8 children records did not have immunizations on file.

The last documented lock down drill was completed on 2/24/22

The lunch menu did not reflect what was served for lunch on today 1/23/23. Note: menu was updated to show what was served for lunch

The last documented severe weather drilll was completed on 2/24/22.

The liability insurance expired on 12/01/2022.

The last documented fire drill was completed on 10/12/2022.

Based on information obtained during the investigation interviews there is a sufficient amount of evidence to suggest that a child was hit with a hand while in care.

A caregiver present does not have a cleared background check. Caregiver stated she has been present since 03/03/2022.

A caregiver present with 7 infants does not have a cleared background check.

First aid kit was lacking antiseptic and cotton balls. This was corrected at inspection when the director placed cotton balls and antiseptic in the first aid kit.

Pre-K schedule is lacking 90 minutes of moderate to vigorous play. This was corrected at inspection when director changed the daily schedule .
