A follow-up inspection was conducted and compliance was not met. This deficiency is being recited. The primary caregiver is not operating the child care home in compliance with Minimum Standards rules and laws. The home continues to be in out of compliance with child/staff ratios.

Individual cloths/wipes are not being used to clean children during diaper changes. Children are being splashed with water from the sink while lying on the counter top.

Children were observed napping on the sofa and blankets on the floor. Children must nap on a cot or non-absorbent mat.

The playyard was not sanitized prior to placing a different child in it.

A completed admission form was not avaialble for two children.

Children 18 months old to 4 years old were observed sleeping on the sofa and not a non-absorbent cot or mat.

The primary caregiver is not current with CPR and First Aid training. The training expired in November 2024.

The home was not equipped with enough cribs/pack n' plays for each non-walking infants. There were four infants in care and only one pack n' play being used.

Two infants were observed sleeping in restrictive devices.

Several barrels that are collecting rain water are accessible to children. Pictures were taken.

A child was observed being fed their bottle with it propped up by a blanket.

Additional padding was in the play yard while being used by a child in care.

The outside area contains many hazards such as lawn equipment, containers of gasoline, paint, bug sprays, car batteries and old tires. Pictures were taken.

School-age children were playing random games on their electronic devices.

Diapers are being changed in an area where children play.

Medications were accesisble to children in care.

The infant is being placed on a pallet on the floor to nap and not the play yard.

The primary caregiver received 25 of the 30 clock hours of required training for the August 2022-August 2023 training year. The caregiver will need to receive 5 clock hours of make-up training.

The home is not equipped with age-appropriate equipment for the children such as musical toys, blocks, trucks, dolls, shaking toys, rattlers, puzzles, etc.

The diaper changing surface is not being sanitized after each use. The primary caregiver is only using water to clean the diaper changing surface.

The primary caregiver is not documenting that fire drills, severe weather drills, checks of the smoke and carbon monoxide detector are being done.

A child under the age of 2 was watching television.

Of the four children's records evaluated, two lacked a health statement from a health care professional.

Children 4 years old and younger are not napping on a non-absorbent surface like a mat or cot. The children are napping on pallets on the floor.

Children's records were reviewed and the following infromaiton was missing: 1.. Three lacked required infromation for an emergency contact. 2. One lacked the admission date. 3. One lacked authorization for emergency medical care. 4. One lacked the name of the school the school age child attends.

Primary caregiver was observed changing diapers in the area used for eating, napping, and playing.

During the inspection the operation was out of ratio by 2 children. There was 8 children in care ages ranging from 4 months to 4 years old.

Fire extinguisher was observed not full.

During the inspection children were observed running around the room, some were sleeping, and one was crying and wondering around the house.

During the inspection it was observed that the two children who had diaper changes, did not have their hands washed.

Gas stove outside was observed with a small flame and there was a strong gas odor in the area.

The outside area does not have 80 square feet of space per child. The outside area has numerous tools for gardening, several large cooking equipments, plants, and refrigerators.

It was observed that children did not have planned activities. Throoughout the inspection children were observed watching television. There were no educational toys observed for children to play with, and there were only four teddy bears.

During the walkthrough two children( 4 months and 3 years) were found hidden in the restroom. Note: The children were removed from the restroom and placed in the daycare area. It was observed that older children were sleeping in restrictive devices.

One out of 8 child records reviewed was missing admission information including immunization records.

Bottles were observed not properly labeled with the child's first name and initial of last name.

Infant was observed in an restrictive device for a duration of an hour.

Children were repeatedly left unattended throughout the inspection.

It was observed that the caregiver did not wash hands after changing two diapers.
