One electrical outlet in the sleeping area was not covered. This was corrected during inspection.

Lase monthly fire drill documented was in 8/4/23/2023.

There was no copy of photo identification on file for provider.

As of today September 2nd, 2022 the operational policies still do not include promotion of physical activities. The following information is still missing: (8)Promotion of indoor and outdoor physical activity that is consistent with Subchapter F of this chapter (relating to Developmental Activities and Activity Plan). Your policies must include: (A)The duration of physical activity at your home, both indoor and outdoor (B)The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities. (C) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor time.

The annual product certification has not been completed since 2019.

The primary caregiver is missing 13 hours of annual training. An evaluation period between 8/23/20-8/23/21 was evaluated and only 17 hours was completed.

A caregiver was observed alone with children during the inspection and the provider has not obtained the eligibility notification from CBCU. Note: It was discussed that this caregiver must not be present until the results from CBCU have been obtained.

The physician or facility information was missing for 1 out of 3 children's files evaluated.
