Operation did not have a current health sanitation report, last listed as 8/27/2024.

One caregiver was observed with a group of fourteen children ages two-three years old. The caregiver did not have an eligible background check.

A caregiver in the Bunnies classroom was unaware of the number of children responsible for. The caregiver stated nine children were present, however ten children were observed in the room. A caregiver in the Lady Bugs classroom was unaware of the number of children present. The caregiver stated nine children were present, however ten children were observed in the room.

Two caregivers responsible for a group of nine children ages three to eight months were present without an eligible background check. Note: The caregivers were removed from the classrooms during this inspection.

One caregiver was observed caring for ten children ages 18 months-22 months. The specified age group for the room is 18-23 which allows for a maximum of nine children. The caregiver was over ratio by one child.

During this inspection I observed expired medication stored for two children enrolled.

Documentation of the annual certification was not available for review during this inspection.

Records were evaluated for ten children enrolled and the following was determined: Nine children needed an address for the emergency contact listed. One child needed an emergency contact other than the parent. Four children needed a food allergy emergency plan signed by parent and physician. Four children were in need of a special needs statement. One child needed a health statement. One child needed an updated immunization record.

The operations emergency practices were reviewed and showed the last documented severe weather drill occurred on 02/16/2022.

Daily Building and Grounds Checklist were available for the weeks of March 27th and April 10th.

Medication for three children enrolled was not stored in its original container.

Updated feeding instructions are needed for three infants enrolled.

The Emergency Preparedness Plan did not include the following: -Procedures for evacuating or relocating children who are under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency. -The address of the alternate shelter away from the center that will be used as needed. -How staff will evacuate and relocate with the essential documentation including: Parent and emergency contact telephone numbers for each child in care and authorization for emergency care for each child in care

Inspector reviewed over personnel files during this visit. Documentation showing staff hire dates and trainings completed during the most recent complete training year was not available for review.

A four month old was observed seated in a bouncer. The safety straps were not fastened.
