One child's file did not have emergecny medical authorization.

It was found that a child in care managed to run out the front doors and into the shopping strip where the operation is located.

A self-report was not received regarding a child in care who wandered away from the operation.

The afterschool restroom ceiling panel is missing and found behind the toilet.

The wooden fence surrounding the big playground has a wooden board that has fallen and has exposed nails accessible to children in care.

A 5 month infant was placed and found sleeping on their tummy who is unable to roll over independently. There is no sleep exception form for the infant.

A mop bucket with water was found in the hallway accessible to children. This was corrected when the bucket was removed from thhe hallway.

In the toddler room, sharp and hazardous office supplies such as hole punchers, staple remover, adult scissors, and a letter opener were found on the children shelving accessible to children. This was corrected when these items were removed.

The big slide on the big playground has an opening at the top of the slide that could potentially trap a child's hand or clothing when sliding down the equipment.

During the director's absence, a caregiver was left in charge at naptime who did not have access to all essential information and records. This was corrected when the designee and second person in command arrived at the operation.

Three children's records did not have a statement or proof that the parent's received a copy of parents' rights.

3 children observed sitting in a bean shaped table with straps were not observed to be strapped in. One of the seats did not have straps.

One caregiver did not have the training in abuse and neglect and building and premise safety training.

A cleaning solution Lemon Plus was not being used according to manufacture instructions. According to the director, an unknown amount of solution is mixed with a bottle of water. The solutiion is used to sanitize table tops According to the instrcutions, the product is not to be used on surfaces that come into contact with food.

Two cribs in the infant room was were observed to have two mattresses in the crib. NOTE:Corrected at inspection, the second mattresses were removed during the inspection.

2 children being transported did not have parent or emergecy contact information. NOTE:Corrected at inspection,parent contact forms were provided during the inspection

3 cribs used in the infant room were observed to have loose fitted sheets.

A 6 mo old not yet on table food did not have feeding instructions completed by the parent

Emergency medical authorization was not available for 2 children being transported. NOTE:Corrected at inspection, the medical authizations forms were provided during the inspection

3 children with listed diagnosed food allergies did not have an allergy plan of file and available for review,

Several bottles in the infant room were observed to be unlabeled.

3 gallons of milk dated 4/15, 4/16 and 4/16 was observed to be in the refrigerator. One of the gallon with the 4/16 exppiration date was served to children for breakfast. NOTE: Corrected at the inspection. All expired milk was disposed of during the inspection.

The fire extinguisher on one of the vans used was not observed to be secured. NOTE: The fire extinguisher was observed to be in the box and under the passenger front seat.

In the infant and toddler rooms. diaper rash creams were observed to be on top of the diaper changing table in reach of children.

In the bathroom of the toddler room, the ceiling was observed to missing a panel.

An emergency evacution diagram was not posted near all exits used. NOTE: Corrected at inspection, diagrams were posted during the inspection.

The child safety alarm on one of the vehicles being used for transportation was not operational during the inspection NOTE: The operation was informed the van may not be used until the safety alarm has been repaired

5 car seats located on both vans used were observed to be expired. Expiration years were 2016,2017,2018,2020,2022 NOTE:Corrected at inspection. All expired carseats were removed and dissposed during the inspection

A catipliller toy on the playground was observed to be broken and had exposed shear points.

A television on top of a rolling shelf was not observed to be properly anchored.

A caregivers purse and uncovers electrical outlets were observed in the toddler room. Uncovered electrical outlets were observed in the pre-k room

In the toddler room on the diaper changing table disinfectant spray and hand sanitizer was observed in reach. In the pre-k room a bottle of disinfectant spray way observed in reach.

The children product safety certificate was last completed 2/1/23

A 2lb roll of ground beef was observed to be open and uncovered in the freezer.

Two bottles of cleaning chemicals were observed to be stored next to boxed of snack crackers and a jar of jelly

Two vehicles used for transportation did not have have completed first aid kits. NOTE:Corrected at inspection. Missing items were replaced during the inspection.

Two infants did not have current infant feeding schedules.

One of four children records did not have a current immunization record.

All six of the classrooms did not have activity plans available.

The diaper changing mats in the infant and toddler rooms were torn, exposing the underlying foam.

The floor in the infant room was sticky and needs to be cleaned.

Two nap mats were torn, exposing the underlying foam. This was corrected at inspection when the Director disposed of the torn mats.

A caregiver did not know how many children she was supervising during naptime.

Two of the bucket seats at the toddler table did not have safety straps. This was corrected at inspection when the Director removed the seats from the classroom.

A caregiver was using a personal cellphone while caring for children.

An11 month old non-walking infant in the toddler room was asleep on a mat. Note: This was corrected when the director placed the infant in a crib.

In the afterschool room, there were cleaning supplies located near the handwashing sink. Note: This was corrected when the director moved the supplies to a higher place.

On the playground, there are two pieces of wood missing from the fence.
