The operation had four medications that were expired, one Epi-Pen and three Benadryl.

Two caregivers did not have documentation that they received personnel policies.

A play structure on the toddler playground did not have a use zone of 6 feet it was 2 ft away from the bench on the playground.

The operation did not have the required surfacing under and around the merry go around on the pre-school playground.

Four children did not have documentation on the parents receiving the operational polices.

Two children did not have a parent signature on the food allergy plan.

Four caregivers did not have documentation that they received center policies.

The operation had a water holes on the pre-school and school age playground. They was a broken toy container on the school age playground. The operation had a deep area on the school age playground to where the ground was not level and could cause a tripping hazard.
