The operational policies were reviewed and the following policies are needed: -Safe sleep policy for infants from birth through 12 months old. -Promotion of Indoor and Outdoor Activities -Emergency Preparedness Plan -Vaccine Preventable Disease -Procedures for supporting inclusive services to children with special care needs -Preventing and responding to abuse and neglect consistent with requirements listed in 746.501 (26). -Instructions on how a parent may access the Texas Abuse and Neglect Hotline and access the HHSC website. -Instructions on how the parent may access the minimum standards online.

Records were evaluated for four children enrolled and the following was determined: One child needs an updated immunization record. One child needs an emergency contact other than the parent. One child needs a parent signature on the food allergy emergency plan.

One caregiver was observed caring for nine children ages 12 months, 12 months, 15 months, 16 months, 17 months, 18 months, 20 months, 20 months, and 20 months. The specified age group is 12-17 months which allows five infants. The caregiver was over ratio by four children. Note: An additional caregiver was added to the room during inspection.
