Five caregivers and the director were missing annual trainings. All caregivers and director were missing hazardous material and emergency prepredness. Caregivers dealing with infants were missing SIDS shaken baby and brain development training.

Smoke detector/carbon monoxide detector testing was not current and last performed July 2025.

One child did not have a health statement from a medical professional.

Operation has not reported their staffing changes for the year 2024 by the due date of 1/15/2025.

A subject was observed at the operation and caring for children in the 1 year old classroom without an initial background check. Note: The subject was removed from the classroom immediately and the Director entered an initial background check submission during the inspection.
