During an inspection conducted for the investigation on 1/17/24, it was found that the Pre-k class for the 3- to 4-year-old children utilize the big playground that has no fence enclosing the play space. The big playground is located next to an open parking lot that has an entrance and exit gate that is kept open for parents and visitors.

Based on information and documents obtained for the investigation, the guardian was not provided with an incident report to sign and date regarding their 3-year-old running into the parking lot from the unfenced playground.

Based on the investigation findings, proper supervision is not being maintained at the operation.

During the investigation, it was found that there was no written notification to licensing regarding a change in added outdoor space for a playground.

A self-report was not made to licensing reporting a situation where a 3-year-old ran into the parking lot from an unfenced playground.

Based on the investigation findings, 3 new hires have not completed their orientation training within seven days of employment.

Based on investigation findings, it was found that a 2-year-old toddler would be in the infant room during naptime with the youngest infant being 4 months old. It was also found that infants under the age of 17 months have been combined with children 18 months and older during the morning intake with 27 children in care from the ages of 4 months to 4 years old.

In the infant room, there were two 3 year olds combined with a younger infant of the age of 4 months old. The oldest a child could be in the room is 22 months old, but there were two 36 month old children. This was corrected when both children were removed from the room and brought to the Pre-K room.

Based on the documentation obtained during the investigation, 2 children's admission forms did not have an admission date documented.

Based on the information obtained during the investigation, it was found that a child was placed in a dark restroom as punishment.

A person identified as a volunteer had unsupervised access to children in care in the infant room without having pediatric first aid training. There were 3 infants in care of the ages 3 months old and two 17 months old.

A 3 month old infant was asleep in a restrictive device. This was corrected when the 3 month old was removed from the device and placed in a crib.

During naptime in the toddler room, a christian video was playing while the children were falliing asleep on the mats.This was corrected when the video was turned off and music was playing.

Requested files were not made immediately accessible for review. Files were requested at 11:30AM and were made available to review over the timespan of 5 hours. The director was assisting in and out of classrooms and assisting with lunch and nap time before providing licensing with the requested files. One caregiver file was never made available for review.

A person identified as a volunteer caregiver was counted in child/caregiver ratio while not having current CPR/First Aid training. This person was also found asleep while caring for 3 infants of the ages 3 months old and two 17 months old. The 3 month old infant was found asleep in a restrictive device. This person was identified as a volunteer by the director, but has been present at the operation consistently. There are no training records for this person.

In the infant room, two bottles of cleaning supples were on the low ledge of the window making them accessible enough to walking infants. This was corrected when the caregiver removed the cleaning supplies by placing them in a locked cabinet.

A person who was identified as a volunteer caregiver was found asleep in the infant room while one infant was asleep in a restrictive device and two infants sleep on mats. This volunteer was counted in ratio and left alone to care for infants. This was corrected when the caregiver was asked to wake up and remove the sleeping 3 month old infant from the restrictive device.

The operational policies or parent handbook does not state that parents may visit anytime during operating hours without requiring prior approval. In the parent handbook, it states that parents have to acquire prior approval before touring and visiting the school.

Based on documentation obtained for the investigation, it was found that one caregiver's training certificate shows inaccurate date information. This certificate counts a total of 30 clock hours completed on 7/8/23. There is a breakdown of clock hours for each training given on the certificate, but dates are not listed for each training as 30 clock hours cannot be completed in one day.

Based on documentation obtained during the investigation, records showing that a volunteer has been constantly working at the operation from April 12, 2022 without having started or completed 24 clock hours of annual training. This volunteer was counted in child/caregiver ratio and has cared for children unsupervised.

One out of four children did not have current immunization records on file.

There was no record of lock down drills.

Two out of three caregivers were missing their educational qualifications on file.
