Toddler room was seen with a peeling baseboard. The bottom of the wired fence has sharp edges poking out and is in reach of the children.

2 children with diagnosed food allergies did not have a plan on file.

The pre-k room was not observed to have enough toys to prevent competing.

11 month old was observed to be asleep in a bouncer for approximately 5 minutes, caregivers pointed towards the child and walked passed them several times before removing the child and placing them in a crib.

Director did not have hazardous materials or food allergy response training.

Two caregivers without documentation of previous experience did not have 24hr of pre-service training available for review.

Fire extinguisher was not observed to be secured in the van.

The pre-k room was observed not having intrest areas for the children.

First-aid kit was observed to be missing antiseptic, tweezers, gloves, and a thermometer. Note: corrected at inspection missing items were placed in the first-aid kit.

An infant was observed sitting in a chair unstrapped.

The toddler room was seen with toys out of reach of the children.

Caregiver failed to wash the hands of 2 infants after diapering.

I observed children not washing their hands before snack time.

Two classrooms were observed not having a number chart to match the numbers labeled on the cots. NOTE: This was corrected during the inspection with a chart made to match the numbers on the cots.

One caregiver was observed with 9 children and SAG 0-11m.

An 3m, 7m, 10m, and 11m was observed with a blanket in their crib. The 11m old was also observed drinking a bottle in their crib.

One medication for a child enrolled was not properly labeled with the child's name and date of entry into the facility. This was corrected during the inspection and withdrawn from medication box and given back to parent.

The driver for the facility did not have current transportation training available for licensing to review.

The caregiver left the classroom and opened the door for the inspector.

An electrical outlet in the toddler room was missing an outlet cover. This was corrected at inspection.

The operation's capacity is 48 .There were 56 children in care during the inspection.

There were several rooms that had chipped paint on the walls.

There were 9 children in the infant room, but 8 were signed in. There were 13 children in the 2 year old room and none of the children were signed in. There were 18 children in the 2-4 year old room, but only 13 were signed in. Note: This was corrected by the caregivers during the inspection.

It is determined during an investigation, children were observed to be out of control.

3 children did not have immunization records for licensing to view.

Several classrooms had chipped paint on the walls.

There was a person without a background check from an outside agency providing services to a 3 year child. Note: The background check rules were discussed with the director and the provider left immediately.

The infant feeding instructions for 6 infants in care have never been updated.
