During walk through it was observed of sunscreen sprays and other chemical sprays inside Room 113- 114 classroon-Restroom, accessible to children. Note: Sunscreen Sprays and Other Chemical Sprays were immediately removed from the restroom by (Caregiver) Terrika Grant and locked in a secure storage closet at inspection.

Operation did not notify parent within 48 hours of child(ren) contracting a communicable disease.

Caregiver was observed grabbing and pulling a child leg and arm during nap time.

During the walk through, it was observed there was a strong odor smell of urine, paper towels and toilet all over the restroom floor from the over flow of the trash can inside the Inventors C Classroom. Note: Corrections were made at inspection, director call in another staff to have the restroom inside the Inventors C Classroom Staff immediately started to clean.

Two caregivers did not know who they were responsible for during the insepction.

A caregiver was in direct care of a child alone without an active background check on file.

The caregiver in the Inventors A classroom (PreK age 3 years old) and the caregiver in the Inventors B classroom (age 3 to 4 years old) did know the correct number of children they were responsible for during the inspection.

Based on the preponderance of evidence from the DFPS investigation, there was sufficient evidence that other children were hit by the caregiver.
