Based on the information obtained during the DFPS investigation, the driver who transported a group of children to school, failed to develop proper seating arrangements for a child who is a known sleeper while being transported.

Based on the information obtained during the DFPS investigation, the driver who transported a group of children, failed to ensure all of the children who loaded the vehicle were properly unloaded. Note: The driver deviated from their normal route to pick-up/drop-off another group of children, as a result, the driver was unaware a child was sleeping on the seat of the vehicle and had not been dropped off at school until the child's elementary school inquired about the child's absence.

Based on the information obtained during the DFPS investigation, sufficient evidence available to support the driver who transported a group of children did not have a checklist which included the names of all children that had been transported.

During the follow-up inspection, a five month old was observed sleeping in a bouncer. The caregiver stated the child had been sleeping 10-15 minutes. The caregiver also stated the child had recently finished a bottle and she had to change the sheets on the infant's crib.

Documentation was not available for review to support a caregiver's high school graduation from outside of the United States. Out of the five records for staff evaluated during the inspection, one did not complete their required training by 05/20/2023 (missing 14 clock hours); three newly hired caregivers w/o previous experience, did not completed 8 hours of training before being in the classroom. Hire dates 08/14/2023, 08/21/2023 and 08/23/2023; one caregiver who supervises children under 24 months did not have verification they completed Sids/Shaken Baby and Brain Development Training. The director had completed 30 clock hours of their annual training on/before 07/29/2023.

One infant was observed sleeping on a baby boppy on the floor with cover.

A boppy pillow recalled 9/23/21 was observed to be in the infant room and in use by an infant in care.

The Children's Products Safety certification was last completed 10/2018

Written feeding instructions for five infants had not been updated within 30 days.

Four infants under 12-months old were observed with blankets in their crib. Note: Three of the infants were sleep and covered with the blanket and the 4th was being put to sleep with the blanket observed in the corner of their crib.

A 5 month old infant was observed to be sleep in a baby bouncer with a blanket over them

A safe sleep exemption form was not available for review for a 5-month-old infant observed to be placed on their stomach while the caregiver was patting their back to put them to sleep.

The fire inspection available for review was last conducted on 08/22/2022.

Two infants were observed sleeping in their cribs without the sheets being tight fitting.

The current gas inspection available for review was conducted 03/16/2020

Based on the information obtained, it is determined that there is evidence to support that the the operation failed to report the incident placing a child at risk.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

The facility owes a total of $26 for background checks submitted. Note: A $16 invoice was sent on 06/02/2021 and another invoice for $10 was sent on 09/02/2021.

The staff training records were not available for review during the inspection.
