2 caregivers in charge were not aware of the ages of the children in their care. One caregiver began to state the ages of children but then began to ask each child their age. One child was asked how old their were in Spanish and the child did not understand the questions.

Rat droppings were observed in the glove compartment on of the vehicles used by children. The contents of a the first aid kit were loose in the glove compartment and lying on the droppings.

Screen time was not observed to be related to a planned activity on the written plan. NOTE: None of the activities written on the plan included screentime.

Fire and shelter in place drills were not properly documented. Fire drills did not incidicate the start and end time of the drill. Shelter in place drills did not indicate they type of drill incidated. It is doucmented that a shelter drill was conducted each year 2025 but only 4 fire drills were conducted in 2025. For the 2026 period only 1 fire inspection is documented

First aid kits on 2 of the 3 vehicles used were incomplete. Both were missing antiseptic solution and one did not have a working thermometer. NOTE: Corrected at inspection All items were placed in the first aid kit

During the duration of the inspection, caregivers were heard from another room loudly telling children to be quite and watch tv. At one point during the inspection, a caregiver was heard telling a child to be quiet then continued by stating" you talk, talk talk,talk talk too much." The caregiver "shushed" the child then used their fingers to gesture to the child to zip their lips twice. Later, the caregiver was heard "shushing" children again putting their finger on their lip and telling children to be quiet.

15 child safety seats were observed to be expired on 3 of the vans used to transport children. 1 car seat with back, expired 2015. 14 booster seats between 2013 and 2022.

Upon arrival, at 6:49 children were observed watchinig tv. Children were still observed watching tv for the duation of the inspection until the last child left at 8:51pm, exceeding the one hr limit for screentime.

There is nothing in the operational policies that stated that a child will be disenrolled if any child has no footing on.

From the video caregiver who release a child did not verify the identity of that parent before releasing a child to another parent.

A caregiver who released a child to a different parent did not ensure the well being of that child.

One caregiver counted in ratio in the infant room has not received eligibility before being allowed to be in the classroom with five infants. Caregiver was removed from the classroom.

There was no proof of education on file for one staff record checked.

There was no record of preservice training on file for one caregiver who was counted in ratio in the infant room.

I obseved that two infants feeding sheets where not updated.

One caregiver counted in ratio in the infant room did not have current first aid and CPR training.

Acknowledgement of orientation was missing on file for one out of four staff records checked. This was corrected during inspection.

Current first aid and Cpr training was missing on file for two out of four files checked.

Date of admission was missing on file for one out of eight fiiles checked. This was corrected durig inspection.

Date of employment was missing on file for two out of four files checked. This was corrected during inspection.

Proof of education was missing on file for two out of four records checked. This was corrected during inspection.

The annual transportation safety training needs completed by the director.
