20 children were combined in one classroom including infants. The youngest child was 12 months and the oldest child is 4 and the specified age group was 3.

Multiple plug sockets did not have a safety cover and one outlet was missing a cover and exposing the entire outlet

Two infant bottles were not labeled This was corrected during the inspection when the caregiver wrote the children's names on the bottles

The director informed me that the children have not gone outside in two months due to the playground area not being safe and currently being remodeled

Three infants not yet eating table food did not have a written feeding instruction sheet.

A child under the age of two was observed watching TV This was corrected during the inspection when the caregiver turned off the TV

A three month old was observed sleeping in a crib with a blanket instead of a fitted crib sheet that fit to the mattress. This was corrected during the inspection when I observed the caregiver remove the blanket and replace it with a fitted sheet.

The maintenance procedure checklist was not available for review.

One out of four caregivers did not have there certificate in CPR.

Fire inspection report was not available for review.

Two school age children were observed playing with personal tablets in the main room with a mixed age group. The activities on devices were not related to a planned activity and the caregiver was not aware of what the children were doing on their devices and the caregiver stated the parents wanted them to be on there tablets because it is summertime. The other children were also watching TV that did not relate to the curriculum and planned activities. This was corrected during the inspection when the director turned off the TV and put the children's tablets away.

Two children with a medically diagnosed allergy did not have a food allergy emergency plan in there files.

Two children were observed eating breakfast while the TV was on. This was addressed during the inspection and the director turned off the TV.

The use zones on the play structure are not a minimum of six feet in all directions.

Two out of four children did not have health statements available for review.

The playground had a wooden board with a nail sticking out.

The playground area does not have a second exit.

The slide on the playground structure does not meet the six feet use zone requirements in all directions.

The caregiver did not have access to accurate information showing each child's age.

Infants were combined with children more than 18 months older than them. The youngest child was twelve months old and the oldest child was six years of age.

A large unmounted fan was accessible to children. This was corrected during the inspection when the director turned off the fan and removed it from the area.

Four caregivers did not have the handling, storing, and disposing materials training available for review.

The playground area had weeds greater than eighteen inches.

Two caregivers did not have the required 24 hours of annual training. Both caregivers were missing training in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medications, Preventing and Responding to Emergencies due to Food Allergies, Emergency Preparedness, Preventing the Spread of Communicable Diseases, and Building and Premises Safety. Also missing training in the required topics of SIDS/Shaken Baby.

There were two baby walkers observed in the nap area. Director removed both baby walkers.
