CCR observed a bucket of mop water in one of the restrooms. NOTE: Water was dumped out and bucket was removed from restroom.

CCR observed three booster seats in the operations van that are marked do not use after 2023. NOTE:Corrected at inspection.The carseats were thrown away and new carseats were purchased at inspection.

CCR observed both dead and living pest in the operations kitchen area including inside the fridge, freezer and on the shelf where food is stored. NOTE: Pest control was last at the operation May 2026

A new first aid kit was observed on the operations van, however there were no scissors nor thermometer. NOTE:Corrected at inspections a pair of scissors or thermometer were added to the kit.

CCR arrived at the operation and observed a caregiver in the hallway, who also allowed us access in the building. As CCR begin the monitoring walk through, the same caregiver was watching a class alone. When CCR asked the caregiver who was watching the children while they opened the door, the caregiver advised CCR that another caregiver was watching the children. CCR was able to confirm with the director that the caregiver who was suppose to be watching the children at that time was not present and did not report to work until 2pm.

It was observed in the restrooms that urine and feces were not disposed/sanitized of from the portable toilets.

A caregiver with a provisional background check with conditions stating that they may never be alone with children, was observed to be transporting a child alone during the inspection.

A child was observed being transported in the operations van, upon exiting the caregiver did not use the alarm. When asked to test alarm the caregiver used the key and was not aware of the button behind her on the door.

CCR observed a substance from the first aid kit located in the operations van had leaked out causing other materials in the kit to turn brown.

CCR observed two booster seats in the operations van that are marked do not use after 2023.

CCR observed the fire extinguisher in the operations van to be empty, the pin had been removed and residue was seen in the passenger side door.

It was observed that the Health and Sanitation inspection was last completed June 2, 2025.

Two caregivers that provide transportation, did not have any documented training.

The operation did not have current CPR training for two caregivers. The only CPR training available to view expired 11/2023.

CCR only observed one exit gate was in the outdoor play area.

The operation didn't have any past or future field trip transportation forms available for CCR to view.

A three year old was observed in the infant room with a 10 month old child. There were 13 children in the building, the difference of the two is 18 months difference in age.

The operation did not have current first aid training for two caregivers. The only First Aid training available to view expired 11/2023.

Notarized affidavit was missing on file for two out of four records checked.

First aid and CPR training was missing for three out of four staff records checked.

Proof of education was missing for one out of four staff files checked.

Date of hire was missing on file for two out of four staff files checked.

One file checked did not have the acknowledgement that she received the policies, orientation and preservice trraining.

Licensing has not received any paper work for the new designated director.

One caregiver was in charge of 10 children ages 15 months to four years with the specified age of 15 months. Classroom was out of ratio by five children.

The caregiver in charge came in 30 minutes into the operation.

A person with an ineligible background check was allowed to be present at the operation after notification had been sent.

We have observed that your Franchise tax is delinquent.

All Licensed childcare centers must annually report to CCR the number of employees who left employment the previous year by January 15. The deadline for entering the number of employees who left employment in 2023 is 1/15/2024. If you have not already you must log in to your childcare regulation account and report the number of employee that left your employment in 2023

Our record is showing that background check fees for the amount of $2 has not been paid.

Immunization record was missing on file for one out of four children files checke.

Physician information was missing on file for one out of four children records checked.

Health statement was missing on two out of four files checked..

Emergency Medical authorization information was missing on one out of four children files checked.

Caregiver observed in the video did not wash her hands after changing a child's diaper.

Caregiver left seven children ages 19 months to five years unsupervised when she came to the door to open the door for licensing.

Caregiver did not use good judgement when she push a four year old who tried to pick up another child and put in arms way.

On the video obsered caregiver did not wash the hand of a child after diaper change.

Current liability insurance was not available during inspection.

There was no current first aid training on file for one out of four staff files checked.

Two out of ten children's records evaluated did not have up to date immunization records on file.

One caregiver did not have current First Aid certification.

The operation has not completed a Children's Product Safety Annual Certification since 2019. Note- This was corrected during the inspection when the director completed the Children's Product Safety Annual Certification.

One caregiver did not have current CPR certification.

The October 2021 fire drill was not documented. Note- This was corrected during the inspection when the fire drill was documented.

The director was missing annual training in the required topics of Handling, Storing, and Disposing of Hazardous Materials, and Preventing Child Maltreatment.

The two children who are transported did not have emergency medical authorization in the vehicle. Note- This was corrected during the inspection when the emergency medical authorization was added to the vehicle for both children.

Two out of two caregivers who have been employed for more than a year did not have the required 24 hours of annual training. One caregiver had 22 hours and the second caregiver had 10 hours. Both caregivers were missing annual training in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medications, Preventing and Responding to Emergencies due to Food Allergies, Building and Premises Safety, Emergency Preparedness, Preventing Child Maltreatment, and SIDS/Shaken Baby.
