I observed a child by himself in the hallway during my walk through at the operation. The supervision was not maintained in the classroom.

I observed the ceilings with brown stains in three classrooms during the walk through.

The caregivers did not know the number of children they were responsible for.

The required steps to sanitize toys is not being used when a bleach/water solution is the disinfectant.

The required emergency medical transport and emergency treatment authorization information for a child is not in the van used for transportation. This was corrected at inspection.

The sign in and out log does not include the name or initial pf the parent and/or staff that is signing the child in and out daily. Also, a school age child is not being signed out when the child leaves the center to get dropped off at school nor is the child being signed back in when she returns to the center in the afternoon.

A caregiver is not sanitizing the diaper changing mat after each use. Also, when the diaper changing mat is sanitized the required steps are not being completed when bleach/water solution is the disinfectant.

An infant was combined with a group of children where the oldest child in the group was more than 18 months older than the youngest child in the group.

A child was left unsupervised in a classroom momentarily while the caregiver walked to the back classrooms. Video footage was reviewed and caregivers were observed leaving children unattended in the toddler classroom on March 18, 2026.

The director was missing nineteen hours of annual trainings.

The director and one caregiver are not current with the required transportation safety training.

One caregiver is not current with CPR and First Aid Training.

The parent handbook lacked the following information: 1. The criteria that will be used to determine when the weather is to extreme for children to go outside. 2. The abuse and neglect policy lacked information on how the center will increase parent/staff awareness, prevention techniques, coordinating with community organizations and actions parent of a child who is victim may take to obtain assistance. 3. What steps the center will take to provide inclusive services

Of the 10 children's records evaluated, 5 were behind with required immunizations.

A follow-up inspection was conducted and compliance was not met. This deficiency is being recited. Updated shot records are not on file for one child.

The background checks for staff no longer employed by the center have not been inactivated.

This standard was evaluated in regard to a report and compliance was not met. On 6/19/24 there were 13 children, ages 13 months old to 9 years with one caregiver. Infants can't be combined with children that are more than 18 months older than the youngest child in the group.

This standard was evaluated in regard to a report received and compliance was not met. A review of sign in/out logs and staff cards reveal the center was out of compliance on 6/26/24. Six infants were in care, ages 3 months, 6 months, 13 months, 15 months, 2-17 months old; with one caregiver. The specified age group was 12-17 months old. With this specified age group, one caregiver can care for 5 infants. There were 26 children in care ages 1-21 months, 7-2 yr olds, 6-3 yr olds, 7-4 yr olds, 4-5 yr olds and 1-9 yr old with one caregiver. The specified age group was 3 yrs old. With this specified age group, one caregiver can care for 15 children.

Of the 10 personnel records evaluated, all lacked the required pre-employment affidavit.

The emergency preparedness plan does not state how children will be cared for during emergencies.

Of the 10 children's records evaluated, two were not up to date with required immunizations.

The diaper changing surface in the infant classroom is torn and is no longer non-absorbent.

The use zone in the front and rear of the bucket swings is not measuring twice the height of the vertical distance from the swing beam.

The pre-k classroom does not have any interest centers.

A caregiver was present without the operation receiving an eligibility determination.

The following items need to addressed: 1. There are torn cots which prevent children from lying on them safely. 2. The van needs to be cleaned. There are empty chip bags, candy wrappers and debris littering the floorboards and seats. 3. Cracked wooden boards on a piece of equipment needs to be replaced.

A container of Clorox wipes was accessible to children in care.

One staff's education document is written in a foreign language.

A table with seats did not have the required safety straps.

Of the 10 children's records evaluated, two lacked signed documentation that the parents received a copy of their rights.

The daily report does not include the amount of food the child eats.

The storage space for children's belongings is not labeled with their name or a picture.

Staff have not completed an orientation

Based on information from the DFPS investigation this standard was found to be deficient.

Based on information from the DFPS investigation it has been determined a staff used profanity in the presence of children.

Based on information from the DFPS investigation it has been determined children are being hit on their hands as a discipline method by caregivers.

Based on information from the DFPS investigation it has been determined staff did not use good judgment when children were encouraged to fight and the situation was not deescalated.
