The infant classroom with children younger than 18 months of age was reduced to one teacher for nap time. This was corrected during the inspection when the other teacher in the classroom returned from lunch.

The director's file was unavailable for CCR review during the operation.

The director's attendance records were reviewed, and it was found that December was not documented.

CCR observed a 4 months in the crib with a blanket. Note: This was corrected when the caregiver removed the blanket from the crib.

An infant was observed sleeping in a restrictive device. This was corrected when the caregiver removed the child from the sleeping device and placed the infant in a crib.

Four files were observed with incomplete immunization forms.

Two caregivers need to meet the following requirements: pre-service training. -Developmental stages of children -Age-appropriate activities for children -Fostering children's self-esteem -Supervision and safety practices -Positive interaction with children -Preventing and controlling the spread of communicable diseases

Two of the children's files observed had missing emergency contact addresses.

The director received 5 of the 30 clock hours of required annual training for the 8/22-8/23 training year. The director will need to receive 25 clock hours of make-up training that will not count towards the current training year. The following trainings are missing: Child growth and development; Guidance and discipline; Age-appropriate curriculum; Teacher-child interaction; Serving children with special care needs; Child maltreatment; Shaken baby syndrome; Safe sleep; brain development; Emergency preparedness; Preventing and controlling the spread of communicable diseases; Administering medication; Preventing and responding to emergencies; Understanding building and physical premises safety; Handling, storing, and disposing of hazardous materials.

The director and two caregivers are missing the CPR and First Aid Certifications.

A spray bottle with a clorox solution was within reach of children in the pre-k and school age classrooms. This was corrected when the teachers place the products out of reach of the children. A storage hallway with chemicals and cleaning supplies was observed having open access to the school age classroom. There was no door to prevent access to children.

One of the children's files that was observed did not state if the child could swim without assistance.

One of the vans used for transportation did not have a first aid kit. This was corrected when a caregiver was able to provide one and place it in the van.

A ceiling tile was observed missing in the boys restroom of the school age classroom.

Two of the operation's vans does not have a child safety alarm installed.

Although, the required 24 hours of annual training were complete, these specific topis were not completed as required. Two of the four files reviewed did not have the reuired topics for training complete.

The director lacked 13 required training hours.

One infant feeding sheet was not updated for the current month.
