In the pre-k room infants and prek children were observed to be mixed, the age of the youngest child was 15 months and the oldest was 4 years old. There was more than 18 months between the youngest and oldest child in care.

Two caregivers in the nap room were unaware of the ages of children in their group. When asked they stated the youngest was almost 2yrs but not two yet. When asked the child's age in months. They stated they did not know. When asked about the age of the oldest. Both pointed to the same child. One said the child was 5yrs old and the other said the child was either 8yr or 9yrs. NOTE: The ages of children present were 2yrs to 8yrs old. The caregivers did not have information avialable to show the ages of each child in care. One caregiver stated the paperwork available was not updated.

During the inspection, a child was observed standing in the hall and refusing to go in the nap room. After unsuccussful attempts to get the child into the classroom, the caregiver left the child in the hall with no further interaction. The child could not be seen by either caregiver and stayed in the hall for approximately 20 minutes. The director was observed to make attempts to placed the child in the room but was unsuccessful as well. There were multiple time were the child was left unsupervised including one time where the director walked past the child to go to the kitcen Another child was observed running out of the room before the caregiver got up to return the child to the room. While speaking with the director, a child left the room unspervised and was found in the hall by the director. The child stated they wanted water. The director explained they could not leave without permission and returned the child to the group. One child was observed lying down and trying to sleep on a table. Two children were running around and ran into each other and fell on top of another child. Neither caregiver present intervened in any of these observations.

During the inspection, the caregiver was observed to step out of the classroom while children were still inside. CHildren were seen phyically fighting and climbing on top of each other. Two children were seen crawling and walking on furniture. While on the playground, children continued to walk inside the buliding without a caregiver. At one poinit, 4 children walked into the building without the caregiver being aware of it before another caregiver inside escorted the children back outside.

The caregiver in care of 10 children 2-4yrs old was not aware the ages of children in their care. The caregiver informed CCR that the posted list of children was not updated

During the inspection, the pre-k caregiver was heard telling a child one two occasions they would not go outside becuase of their behavior. The caregiver stated that for discipline children sit for 10 minutes and if the whole group is misbehaving all children aare required to have a 10 minute quite time.

A child was observed sleeping on the bare floor. Another child was not observed to have a cot/mat for naptime. According to staff there were not enough for all children. Cots/mats are not kept for drop-in children.

A caregiver with a provisional background check with conditions stating they may never be alone with children was observed to be left unspervised multiple times during the inspection.

Multiple children were observed sleeping on unlabeled napmats. According to the caregiver, the children pulled them off but they had not yet been replaced.

A bottle of chemical cleaner and bleach water spray bottle in the pre-k room and Clorox wipes in the bathroom used by children were observed to in unlocked cabinets in reach of children

A caregiver was observed to be on their personal cell phone during naptime supervsion. CCR stood infront of the caregiver for approximately 30 seconds before the caregiver was aware of CCR's presence in the classroom. Another caregiver present called their name multiple times before the caregiver looked up and acknowledged CCR's presence.

There was no emergency contact information for one out of four records checked.

Current first aid and Cpr was not observed on file for the director.

There was no release information on file for one out of four files checked.

Infant feeding instruction was not updated for two infants. Last update for one was 5/28/25 and the other last update for the second infant was 5/5/2025.

Health statement was missing on one out of four files checked.

Two sealt belt in one of the vans used for transportation needed to be fixed.
