One child's admission information was missing the physician information from the admission information during the inspection.

The immunization record for one infant 9 month 9-month-old was not available for review during the inspection.

The health statement for three children's admission information was not available for review during the inspection.

One caregiver who was hired on September 26th, 2022 did not have their initial background check submitted prior allowing this caregiver to be at the operation. This was corrected when the director entered the background check information for the employee during the inspection.

Three employee personnel file did not have the highschool diploma documentation to review for qualification during the inspection.

The operational policices was observed missing the suspension and expulsion during the inspection.

The annual training documentation for the director was observed missing 12 hours out of 30 training hours required.

Two milk bottles in the Infants Classroom was observed not having the infants's name labeled. This was corrected when the director and caregiver was brought to attention and labeled the names of the infants during the inspection.

Annual training docuementation for one caregiver was observed expired in September 2022.

Three infants feeding instructions was observed have not been recertify since September 2022.

There were no records of the daily playground checklist for the past three months available for review.

The statements showing employees were recevied and understand the personnel policies were not available for review.

One caregiver was observed using their cellphone while supervising children during naptime. NOTE: This was corrected; the caregiver removed their cellphone during the inspection.
