Caregiver did not show good judgment when propping an exterior door open which led to a child walking away from the center. Caregiver did not show good judgement when placing a child in a highchair for activity not related to feeding.

The infant room was used as a passageway by an older child with infants in care.

The director failed to clearly make employee assignments in her absence with a caregiver left in charge who was capable of enforcing the operation's policies and procedures.

A child was unsupervised for about 5 minutes and walked away from the operation.

3 out of 4 children under 4'9" in height and under 8 years old were not seated in a belt-positioning booster seat, safety vest, or harness according to the manufacturer's instructions.

1 out of 5 infants had an outdated infant written feeding instructions form. Note: the caregiver had the parent re-sign and update the form during inspection.

1 infant, 6 toddlers and 1 PreK children did not wash their hands after toileting. Note: this was corrected at inspection when the caregivers washed the children's hands.

One caregiver is still missing an affidavit forms.

The operation had a vendor working at the operation that comes once a week with out a background check ran. Corrected: When the vendor worker left for the day.

Two caregivers are missing affidavit forms.

The director stated she had 14 children when I counted they were 15 children in care.

Doing naptime two children were in another classroom not being supervised by the caregiver.

The fence to the left of the operation on the playground was not 4 feet tall, it was 3 feet 4 inches.

The director had 15 children in care specified age groupe was 2 years old, the director was over by four children.

A child was on their tablet doing the nap time period.

One caregiver did not have CPR training and the other caregiver had online CPR training.

One caregiver was missing first aid training.

The center's emergency prepardness plan needs updating to include language in how caregivers will safely move children under 24 months old to the relocation site.

Multiple pesticides are being stored in reach of children on the playground.

The operation has 3 fire extinguishers rated at 2A--10BC.

The 5'6" slide does not have the required 6 foot use zone around the perimeter of the slide and overlaps the use zone of another piece of equipment.

The backside of the center facing and exposed to the playground is a painted surface with chipping paint and has 2 broken boards in need of replacing. The exterior boards are accessible to children.

A Little Tykes type of playground play equipment has deteriorated on the play surface causing the plastic to rip and become sharp and jagged. Wooden borders in the playground need replacing.

The operation did not conduct a fire drill for the month of June. This was corrected at inspection when the center conducted a fire drill.
