During the investigation inspection, it was revealed that the director/owner exercised poor judgment by allowing children to play on the sidewalk in front of the operation, where they had access to an open parking lot. This area is not an appropriate or safe play space for children to play.

It was revealed that the children witnessed the owner/director disciplining a relative child during operating hours.

The children were taken to the owner/director's home without the parents' knowledge, and the parents were not provided with the specific time or address where their children would be while away from the operation.

It was revealed that the owner/director did not provide the children with shirts or name tags to help identify them during the field trip.

The owner/director exercised poor judgment by failing to follow minimum standards when notifying parents about where their children would be while away from the operation.

During the inspection, CCR observed no running water in the operation building. CCR also observed that the toilet handle broken

CCR observed the children's cot has wear and tear, and is no longer waterproof.

During naptime the operation was found to not have adequate space for caregivers to walk between nap time mats.

Based on the information found during the investigation, there was sufficient evidence to support that a caregiver and the director popping the children when attempting to redirect them as a form of guidance.

An incident in which a child was placed at risk for injury was not reported to CCR as required.

Multiple places in the fence of the outdoor space were broken, bent or severly damaged, exposing children to nails that could impale and holes that could be a cut, pinch or entrapment hazard.

Two children did not either an immunization or immunization waiver on file at the operation.

Full garbage bags were seen in plain view near the front door leading to the outdoor space and accessible to children. NOTE: The deficiency was corrected with a caregiver placing the full garbage bags in the trash bin outside the facility.

Four children's records were reviewed. One child's immunization is missing two doses of the PCV13 vaccine. They should have four doses based on their current age.

Based on a DFPS investigation, the operation failed to notify a parent of a less serious injury sustained while a child was in care.

Scissors and tweezers were missing from the first aid kit. NOTE: The deficiency was corrected at inspection.

There are broken planks within the fenced in space that increase risk to children.

Two of the 6 children records did not have an address for the emergency contact. This was corrected at the inspection when the owner called the parent and was able to get the address for both students.

The caregiver walked to the front lobby area of the center and was caring for the mixed age group of childrenn while leaving the 2 year old alone.

During the follow up inspection, the caregiver with the mixed aged group repeatedly walked out of the classroom leaving the children unattended. The caregiver was observed in the kitchen preparing lunch.

Based on the information obtained during the inspection, it was observed that the infant classroom was out of ratio. There were 5 infants with one caregiver.

During the follow up inspections, the permit holder has not hired enough caregivers and has not made an attempt to mitigate the risk to children in care. The permit holder has not maintained compliance with minimum standards. There have been several deficiencies with repeated violations. There have also been discussions with the permit holder in regards to the issues and how to remain in compliance with minimum standards.

There were 4 infants, aged 6-12 months old, left alone in the classroom with the doors closed, while the only caregiver available was caring for the other children.

I observed the director caring for 26 children aged 6 months through 7 years old. The children were split up in three classrooms: 4 infants, 9 (2-7 year olds), and 13 toddlers were taking a nap.

Based on the evidence obtained during the investigation, there is suifficent information to suggest that supervision is not being properly maintained. There were instances where there is one caregiver providing care to all groups at the center.
