During the recent inspection, it was observed that a few child sleeping mats were torn and ripped, compromising the integrity of the protective outer covering

The operation did not have a current Fire Inspection Report for review.

Two caregivers are missing the required 24 annual training hours.

Two children enrolled in the operation did not have the required Health Statements on file.

During the inspection it was found that the operation is not maintaining current feeding instructions for six infants in care.

The operation did not have two exits out of a fenced area in the playground.

One caregiver first aid expired on 1/2024.

The operation does not have an updated gas inspection, the last inspection was conducted on 9/28/2022.

The operational policies does not cover how they will promotion of indoor and outdoor physical activity and the benefits for the children.

The Tiny Tots classroom had one caregiver with six infants specified age group for 12-17 months old is 1 to 5. The caregiver was one over ratio at naptime.

The emergency preparedness plan did not have how the operation will move infants and children with limit mobility.

One caregiver CPR expired on 1/2024 and another caregiver CPR was conducted online.

A review of the Employees Who Left Employment in 2023 data from January 17, 2024, revealed that the center had not reported the number of employees who left employment in 2023 by January 15, 2024, as required.

The director and two drivers did not have transportation training and have been transporting children.

Two caregivers are missing 2 hours for annual training for training year 6/14/2022 to 6/14/2023 and training year 9/16/2021 to 9/16/2022. Another caregiver is missing 5 hours or annual training for training year 8/16/2022 to 8/16/2023. Corrected: When taken two hours of training for training year 6/14/2023 to 6/14/2022 and 9/16/2022 to 9/16/2023. Five hours of training was taken from 8/16/2023 to 8/16/2024.

Four caregivers did not have the corrected curriculum for CPR it was not instructed on a manikin in person.

Two children in high chairs were not strapped in due to the hair chairs not having straps on them. Corrected: When the operation put straps on the high chairs and the children were strapped in.

The operation did not notify licensing that they were offering transportation as a service. Corrected: When I reviewed the buses and transportation standard.

The director is missing 27 hours for annual training for training year 5-1-2022 to 5-1-2023. 19 hours were taken from training year 5-1-2023 to 5-1-2024 and still missing 6 1/2 hours of annual training.

It was stated by a caregiver she uses hand sanitizers after changing children, the children in the class ages are 13-15 months old.

The operation did not have an updated sanitation inspection, the last inspection was expired on 7/27/2022.

The operation has not had a gas inspection since 10/08/2019.

It was observed that the spacing between cots did not have enough space between the cots for children to get up and move or the caregiver to walk through.

After the caregiver changed a child hand sanitizers was used and the caregiver did not wash their hands with soap and water.

It was observed in the two classroom were children were napping the room did not have adequate lighting during nap time.

The operation last fire inspection expired on 1/31/2021.

The operation did not have a updated children's product form.

Caregiver stated she had 11 children in care, when I counted there were only 9 children.

Nine staff records were reviewed. One staff member is missing 16 annual hours. One staff member is missing 21 annual hours. Director is missing all 30 annual hours.
