Training records for the primary and assistant caregiver were not made available for review during the time of the inspection.

It was observed that screen time was being utilized during a mealtime. This was corrected when the primary caregiver turned off the tv.

One school age child in care was missing school information and contact number in their file. This information was corrected by parent permission during pick up time.

Three children's files did not contain health statement information.

It was observed that cleaning supplies and hair chemicals were found to be accessible to children under the bathroom cabinets.

The outside play area had broken pieces of glass and tile on the ground. The fence was not in good repair and contained broken wood and exposed nails.

Upon arrival to the home, it was observed that children of the ages 3months to 3 years old were engaged in screen time. This was corrected when the screen was turned off.

One child's file did not contain release information.

The outdoor a/c unit in the back of the home did not have a safeguard or barrier around it.

The children were watching a televison show rated "PG" which means parental guidance. This was corrected at inspection.

A follow-up inspection was conducted and compliance was not met. Daily sign in and out logs for all days in the month of June were not available for review.

A pre-employment affidavit was not on file for the assistant caregiver.

An assistant caregiver was left alone caring for children.

An assistant caregiver, hired on 6/2/25, did not have a notarized affidavit available for review in the file.

Based on the information provided, there is a video that shows a 5 year old, placed in a high chair as a form of discipline.

The permit holder admitted that an individual was present and in the presence of children without having a completed initial background check.

Based on the information provided, a photo shows 2 infants left alone a room. One infant was placed in a car seat on the bed and the other infant was sitting in a play pin. Due to the proximity of the room of where the infants were placed, a caregiver would not be able intervene and ensure proper supervision is maintained.

During the inspection, the sign out sheet for the children that were not enrolled into NCI, was not available for review.

Based on the information obtained, the logs show that there were 13 children clocked in and present at the same time. In addition, there were 3 other children who was not clocked in but were also present with the other 13 children in care. A photo also shows there were 14 children present in one room together.

Based on the information provided, a photo shows a total of 4 infants with 10 other children who were at least 18 months and over with one caregiver.

Toddler children and children up to 4 years old are being required to remain in time out for up to 5 minutes which over the one minute per year requirement.

The parents of the two infants have not completed written feeding instructions.

There were uncovered outlets in areas used by the children. This was corrected at inspection.

Of the four children's records evaluated the following information was missing: Three children were behind with immunizations. One child's record lacked an immunization record.

Documentation of current CPR and First Aid training for the primary caregiver was not available for review.

Of the four children's records evaluated, two lacked health statements.

The home is not equipped with a carbon monoxide detector.

One room being used by children is not equipped with smoke detector.
