The operation has not completed the required number of lockdown drills.

The operation did not notify the parents of the safe sleep deficiency by using Form 2970, Notification of Safe Sleeping Deficiency.

Records were evaluated for five staff members and the following was determined: A Pre-Employment Affidavit for Applicants for Employment is needed for five staff members. Four staff members need documentation showing how they meet the minimum education qualifications. Five staff members need a statement of center policies.

Records were evaluated for six children enrolled and the following was determined: Six children needed a health statement Five children needed emergency contact information, Three children needed a special needs statement. Two children needed updated immunization records. One child needed an address for the emergency care center.

Upon entering the infant classroom I observed two children ages seven and eleven months old asleep in rockers.

During this inspection a caregiver allowed two infants ages seven and eleven months to use blankets while napping.

Two staff members who do not have two years of experience have not completed 24 hours of preservice training.

Upon arrival a four year old was observed alone in the pre-k classroom watching television. Note: The caregiver removed the child from the classroom and placed them with the group of children who were seated in the lobby area.

Emergency medical transport and treatment authorization forms were not available for five children who were transported during today's visit.

Current transportation training is needed for two directors and one staff member who transports children.

Pediatric first CPR certificates for one director and a staff member who transports children expired 08/01/2024. Documentation of current pediatric CPR training was not available for an additional director.

The operations transportation binder was reviewed and it did not include parent's names and telephone numbers and emergency telephone numbers for each child that was transported.

A caregiver responsible for transporting a group of five children ages 4-9 years old did not have an active background check. The operation replaced the driver during this inspection.

A twelve month old was served a blueberry pancake during today's visit. The child was not provided a sanitary holder for the pancake.

A current driver's license is needed for one director.

An infant was observed in a restrictive device longer than fifteen minutes during this inspection and was brought to the attention of the caregiver in charge. Note: The child was observed in the restive device for approximately 47 minutes. The director removed the child from the restrictive device upon arrival.

Pediatric first aid training certificates for one director and a staff member who transports children expired 08/01/2024. Documentation of current pediatric first aid training was not available for an additional director.

A four month old infant was observed sleeping in a play yard with a bib and pacifier with attachment. Note: The infant caregiver removed the pacifier and attachment.

The operation is not permitted for field trips. During this inspection it was observed the operation took a total of five field trips in the month of June.

During this inspection I observed two unlabeled bottles in the infant classroom. Note: This was corrected during inspection when the caregiver labeled the bottles.

Records were evaluated for ten children enrolled and the following was determined: Two children needed the telephone number of the school they attend. Two children needed the date of admission. Two children needed a health statement signed by a physician. One child needed an emergency contact other than the parent. Two children needed emergency care information such as physician name, address, and phone number. One child needed authorization for emergency care. A food allergy emergency plan is needed for one child with a diagnosed food allergy. A parent signature is needed on the food allergy emergency plan for one child.

The diaper changing pad in the toddler classroom was torn.

Electrical fans were observed on shelving accessible to children in the toddler and pre-k classrooms.

During this inspection, one caregiver did not have documentation of current pediatric CPR training.
