Orientation training is needed for one staff member.

One caregiver needs pediatric CPR training.

Records were evaluated for six children enrolled and the following was determined: Three children needed a statement of special needs. Two children needed an emergency contact. Two children needed a health statement signed by a health care professional. One child needed field trip permission.

Documentation of safety drills was not available for review during today's inspection.

Attendance records were not available for review for the center directors.

The operation was not completing monthly inspections of the active play space and equipment. Documentation reviewed showed the last checklist was completed 06/2024.

The center directors are in need of thirty clock hours of annual training.

One caregiver needs training in pediatric first aid with rescue breathing and choking.

Two caregivers need 24 clock hours of pre-service training.

The annual certification was last completed 05/01/2024.

An infant crib was not labeled during today's inspection. Note: This was corrected during inspection.

Updated immunization records are needed for two children in care.

Personnel Records were evaluated for five staff members and the following was determined: Two staff members needed a statement indicating they have received the personnel and operational policies. One staff member needed documentation showing how they meet the minimum educational requirement. One staff member needed documentation showing orientation training was completed.

Records were evaluated for six children enrolled and the following was determined: Three needed updated immunization records. Two needed an address for the emergency contact. Two needed a health statement. One needed a special needs statement. One needed a food allergy statement.

A caregiver responsible for a group of children ages 7 months- 12 years old was unaware of the number of children responsible for. The caregiver stated eight were present, however nine were present in the room.

The sign in and out log was reviewed and showed 1 of 15 children were signed in. Note: This was corrected at inspection.

The menu stated the children would be served the following: Chicken, Bananas, Sweet Potato Fries, and Milk The children were served chicken, rice, cucumbers, and milk. Note: This was corrected during inspection.

-The door to the men's restroom was off the hinge. I observed nails sticking out of the area. -Tarp was observed protruding from the loose fill surfacing, presenting a tripping hazard.

Based on the information obtained, it was determined a caregiver responsible for a group of children ages 3-5 years old did not maintain supervision when a child was able to pull out another child's hair.
