There was no current Fire inspection Report available for review during the inspection.

There was no current Health Inspection report to review during inspection. The Health Inspection report was observed with expiration of 8/27/24.

A caregiver was present in the infant classroom caring for 6 children without a cleared eligibility check.

Based upon the information gathered during the investigation, a caregiver was observed on video hitting a child on the hand and pushing them on the carpet. The video also showed the caregiver hitting another child in the mouth.

Based on the information obtained during the investigation, there is suffient evidence to determine two caregivers were in a verbal alteration infront of the children in care.

The director annual training was not available for review.

Five out of five caregivers training record was not available for review.

Observed five children sitting in table chairs without the safety straps fastened. Four of the table chairs children were sitting in were missing the straps. Note: Two of the children were strapped in properly and the other three children were taken out of the table chairs that were missing the straps and placed in high chairs. The Director placed an order for new straps during the inspection.

The following is missing from the emergency preparedness plan: (b) How children will be evacuated or relocated to the designated safe area or alternate shelter, including specific procedures for evacuating and relocating children who are under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing impairments.

A caregiver's record evaluated is missing 11 annual training hours.

The operation does not have a complete and detailed emergency preparedness plan.

Three children records evaluated were missing the special needs acknowledgment from their admission form.

Observed missing electrical outlet covers from a wall outlet in the 2 year old classroom. Also, observed a missing outlet cover from a wall outlet in the 3y-4y old classroom. Note: Outlet covers were placed in the outlets during the inspection.

The operational policy handbook does not consist of details regarding the promotion of indoor and outdoor physical activities. It should include the following: (A) The benefits of physical activity and outdoor play; (B) The duration of physical activity at your operation, both indoor and outdoor; (C) The type of physical activity (structured and unstructured) that children may engage in at your operation; (D) Each setting in which your physical activity program will take place; (E) The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities; and (F) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.

Observed three cribs with loose-fitted sheets in the infant room.

The operation's first aid kit is missing cotton balls and a thermometer. Note: The items were placed in the first aid kit during the inspection.

The director is missing 15 annual training hours.

The operation is not practicing fire drills monthly. There is no documentation to show a drill was practiced in January 2022.

Observed an infant bottle in the classroom's refrigerator that was not labeled with the child's name. Note: The bottle was labeled with the child's name during the inspection.

A caregiver's record evaluated is missing 11 annual training hours.

The diaper changing pad in the schoolers classroom contains rips/tears in it. The caregiver stated the changing pad is used for a special needs child.

The operation does not have recent documentation of the building and grounds maintenance checklist on file. The checklist made available was from December 2021.

There is a door to an electrical room in the girls restroom and a door to another electrical room in the boys restrooms that are not locked nor has a safeguard to prevent children from going into the electrical rooms.

A caregiver did not wash a child's hands after changing the child's diaper. Note: The caregiver washed the child's hands when prompted to do so.

The operation failed to report two employees by January 15th who are no longer employed at the operation. The two employees background checks are currently active and are due for renewal.
