Operation has not reported their staffing changes for the year 2024 by the due date of 1/15/2025. NOTE: Director provided proof of compliance same day.

It was found that the caregiver did know how many children the caregiver was repsoblbie for when outside as caregiver failed to do count before leaving the classroom and when the classroom got outside.

It was found that a child was left unattended in the classroom for 2 to 3 minutes.

Based on the information obtained, the operation did not follow the background check conditions set by CBCU for a person with provisional eligibility by allowing the person to oversee staff.

Based on the information obtained, the operation is not in compliance with minimum standard rules. The provider allowed a person to be in charge of the after-school program who did not meet all qualifications.

Based on the information obtained, a caregiver has placed their foot over a child for them to go to sleep.

Date of admission was missing on file for four out of ten children files checked. This was corrected during inspection.

Four children records evaluated were missing one or more of the following information from their admission forms: -Special needs Acknowledgment; -Date of Admission; -Address for child's emergency contact person

Four staff members were observed at the operation and providing direct care to children without an initial background check submission entered. One staff member was observed in a classroom. One staff member was observed in the outside courtyard. Two staff members were observed on the playground. The four staff members were assisting other staff that has completed background checks on file.

The first aid kit is missing the guide to first aid and adhesive tape. Note: Corrections were made during the inspection.

The operation's emergency preparedness plan is missing the following from this subchapter: 1(d)-an emergency evacuation and relocation diagram. 1(e)-the address of the alternate shelter away from the operation.

The operational policy handbook is missing the procedures on how parents may contact the child care licensing office, access the Texas Abuse and Neglect Hotline, and access the HHSC website. Note: Correction was made during the inspection.

Four staff records evaluated were not maintained and were missing the following: -Documentation showing the dates of the first and last day on the job. -Documentation showing how the employee meets the minimum age and education qualifications. -A notarized Licensing Affidavit for Applicants for Employment form. -A record of training hours. A statement signed and dated by the employee showing they have received a copy of the child-care center s: (A)Operational policies and (B)Personnel policies. -Proof of request for background checks -A copy of a photo identification. -A statement signed and dated by the employee verifying the date the employee attended training during orientation that includes an overview of your policy on the prevention, recognition, and reporting of child maltreatment.
