The director did not have the transportation training certificate for review upon request during the investigation.

Based on the information obtained from the investigation, it is determined that the operation is not in compliance because the plan to handle transportation emergencies was not provided when requested.

Based on information obtained during the investigation, it was determined that the daycare van was not in compliance because the operation did not maintain current liability insurance and the vehicle registration for the van used to transport children.

There were no first aid kit observed in the bus that was used to transport children. A new First Aid kit was added to the bus during the inspection.

The immunization records for an infant 7 months old was not available for reviewed during the inspection.

The transportation training documenation for the director was missing one hour as required, the certficate reviewed showing only one hour was completed in January 2024.

The playground maintenance check list was not available for review during the inspection

The infant, toddler and PreK room had brown stained ceiling tiles.

Two out of five children files did not have immunizations available for review during the inspection.

Based on the information gathered during the DFPS investigation, there is sufficient evidence the director threatened corporal punishment when using a ruler with a relative child in care.

Six children were observed watching a children show on the iPad during the walk through of the investigation inspection. Three children were observed to be under 2 years old. This was corrected during the inspection when the caregiver in charge was informed and removed the iPad.

Based on the information obtained from the investigation, there is sufficient evidence to show a three years old child was left unsupervised in another classroom while in care at the operation.

A caregiver from Infant classroom did not wash their hands after finished changing diaper for a child before move on to the next child. This was corrected when the caregiver was informed and washed their hands after each diaper changing period.

A substitute caregiver did not have the current training certification to reviewed, The last training certification was observed completed in 2020.

The admission information for one infant10 months old was not available for reviewed.

The immunization records for two infants was observed not available for reviewed.

The feeding instruction for four infants in care was obseved not current during the inspection. There were no certification date from three of the feeding intruction sheets from the Infant classroom.

One child record was not available for review upon request.

The sleeping mats in the Pre-K classroom were observed being torn showing the filling which is no longer waterproof. This was corrected, new nap mats were availble to replace the damaged ones.

One caregiver was missing eight hours of pre-service training certification before they can be counted in ratio.

The operation failed to comply with Minimum Standards when children were combined past the required age gap. NOTE: This was corrected; the groups of children were separated.

Two classrooms were observed being out of ratio during the inspection.
