During the walkthrough I observed infant bottles unlabeled. This was corrected when operation staff labeled the bottles.

During the inspection it was observed that a child's diaper was changed and the caregiver did not have the child was their hands after.

During the inspection it was found that the children's product annual certification had not been completed since April 2024.

Druing the inspection it was observed that a 15 month old was given a whole grape.

During the inspection it was observed that a caregiver was changing diapers, one child after the other, and not sanitizing the changing table between each child.

During the inspection it was found that both of the transportation vehicles did not have antiseptic solution available for use.

During the inspection it was observed that both of the electronic safety alarms were turned off in the transportation vehicles.

During the monitoring inspection it was found that the pre k 1 class only had one outdoor play opportunity scheduled each day.

While reviewing files during the monitoring inspection it was found that an employee did not have their pre-affidavit form 2912.

3 caregivers and the director do not have current CPR training with the use of a hands on manikin. 2 caregivers have CPR training through the NationalCPRFoundation which does not use a hands on manikin.

A 6 month infant was observed being swaddled while laying in a crib. This was corrected when the caregiver removed the swaddling blanket. Photo was taken.

Based on the evidence found during the DFPS investigation, a caregiver did prohibited discipline when she grabbed a child by the wrist resulting in a serious injury.

Based on the Preponderance of evidence found during the DFPS investigation, a caregiver was found to have Physically Abused a child in care.

It was found that the health inspection expired 4/19/23. The inspection was last completed on 4/19/22.

A 7 month old infant was placed a sleep inside the crib with the bib on. Note: This was taken off at inspection.

Based on DFPS investigation it was found that the incident report was not completed concerning when a child was not appropriately supervised by a center employee.

Based on DFPS investigation, after reviewing the camera footage it was found that the caregiver grabbed and dragged the child from the playground.

Based on DFPS investigation, it was found that the caregiver did not know how many children she was responsible for, resulting in leaving a child outside unsupervised.

Based on DFPS investigation completed, it was found that a child was left unattended outside for 4 minutes. Child had walked to the front of the office to get inside.

Activity schedule shows Pre-k are conducting only 60 minutes of moderate to vigorous play.

Operational policies are lacking information regarding promotions of physical activity.

Caregiver stated that infants do not go outside.
