The inspector reviewed the facilities list of children that are normally transported each day. The inspector observed that one child that was normally being transported did not have the emergency medical transport and treatment authorization form. NOTE: this was corrected at inspection. The director made copy of the enrollment form and put it in the binder.

Based on the information obtained during the DFPS investigation, there was sufficient evidence that the operation failed to provide an incident report for a child who was bitten on the hand.

Based on the information obtained from the DFPS investigation, at the time of the operation visit on 06/20/2025, CCI observed that no designated person in charge was present at the center.

Based on the information obtained from the DFPS investigation, roaches were observed in the building.

The operation did not intervene when a parent took the child out of the classroom to another classroom, even though she was not on the pick-up list for that child.

Based upon the information gathered during the investigation, there was sufficient evidence to support the allegation that a caregiver used inappropriate discipline. The caregiver took 2 children outside of the building to scold them.

The first aid kit in the van used to transport children does not have cotton balls,scissors., and thermometer. The operation's director included all required items in the first aid kits during the inspection.

Out of the eight children's records evaluated, one did not have immunization records available for review and one did not have up to date immunizations required by TX State Immunization law.

I observe caregiver present that was missing current background check.

There were broken toys, small hole along the sidewalk and unsecured fencing present in the playground area.
