Based on information gathered during the investigation, one caregiver did not exercise good judgement by using profanity in the presence of children in care.

The inspector observed one teacher that had left 2 toddlers and 2 infants on highchairs from 3:05 PM to 4:23 PM until the owner stepped in to remove the children out of the highchairs to participate in other activities. These four children had finished their PM snacks at 3:15 PM.

In one classroom, the daycare combined 2 infants with 9 toddlers for the entire day. The total children in the daycare at the time was 23 children. Then the total became 42 children when the school aged children arrived.

The daycare has not submitted document for the new director who has been worked since last Tuesday 07/08/2025. Note: corrected. The daycare submitted documents for the new director at inspection.

1/There were water standing till water areas in the playground which were having mold and active worms in them. 2/There were some broken chairs and water leak from the ceiling in beam room using by children. 3/There were some rotten chairs and torn basket net for basket ball hoop on the playground using by children. Note: all broken and rotten chairs removed out of beam room and playground. Pictures taken and showed person in charge.

The sanitation inspection expired on 08/09/2024.

Personnel records are not available for review during hours of operation for each employee, caregiver, substitute.

1/the playground posing entangle a child's clothing. The daycare will not use this equipment for children in care until they are fixed as required. 2/The paint on the 2 exits doors from the Science Lab and Pre K classroom was peeled off and accessible to children in care. Pictures taken.

The center did not report to Licensing the direcror absence and who meets director qualifications to serve during the director's absence as required.

All children's records are not available for review and/or reproduction by Licensing upon request during hours of operation.

The infant classroom had 6 infants and 1 caregiver. The infants were 6, 9, 13 , 15, 16, and 18 months old. The caregiver should of only had 5 infants.

Based on evidence gathered during the investigation there were findings of live pests in the operation.

The caregiver in infant room did not know the ages of children in her care and not having information showing each's child age during the investigation. One six-month-old child did not have her name and age on the list. Picture of the list was taken.

The daycare admitted the child on 03/15/2023 but the admission information signed on 03/16/2023 by parent.

There were some cracks on one barrow train and one yellow slide on the playground posing entangle a child's clothing. Pictures taken. The daycare will not use these two equipment for children in care until they are fixed as required.
