The operation has not had an annual fire inspection and the last fire inspection was conducted on 4/12/2025 and the reinspection was conducted on 5/02/2025

Based on the preponderance of evidence there is sufficient evidence to support the claim that the caregiver is using harsh language when communicating with the children during diaper changes.

One out ten caregivers did not have a first aid and rescue breathing available for review

One out of ten caregivers did not have a CPR available for review.

One caregiver was supervising 15 toddler children by themselves. The specified age group was 2. This was corrected during the inspection when another caregiver came to assist.

The following documentation was missing from staff files evaluated: - Certificates of Current CPR and First Aid Training - Pre-Employment Affidavit Forms -Proof of High School Diploma or Equivalent

Documentation showing staff have completed 24 clock hours of training was not available to review during the inspection for 1 staff file evaluated.

The director who has been employed for less than a year did not have proof of orientation. This was corrected when the form was completed during inspection.

Documentation of the most recent severe weather drill was not available for review during the inspection. Note: The provider documented the severe weather drills during the inspection.

2 out of 10 staff files evaluated did not contain proof of education.

The operation failed to report suspected abuse and neglect of a child in care to DFPS.

Based upon the information obtained from the DFPS Investigation, a caregiver was aggressive and forceful to a child in care resulting in the child obtaining scratch and nail marks.
