I did not observe any monthly fire drill for this operation since the begining of this year.

I observed a college student assisting a caregiver who has no background check entered under the operation. Operation must receive notifiation from the CBCU that the subject of a background check is eligible before allowing the suject to be present at your operation.

A two years old was on a high chair with out a stafety straps. Child was removed and placed on the floor.

The system is showing that background check fees for the amount of $6 has not been paid. This was corrected during inspection when owner paid online.

Six children were present during inspection but five were signed in. This was corrected during inspection.

One caregiver in charge of children during inspection was not listed in the current employee list. This was corrected during inspection.

Fire extinguisher was not mounted in the new vehicle used to transport children.

The new van now used for transporation has no child safety alarm installed according to manufactures instructions.

Two classrooms were not supervised during naptime. Caregiver was in the after school room with one child on her lap.

There was no acknowledgment that four staff received any operational policies.

Notarized affidavit was missing on file for four out of four staff files checked.

There is also no acknowledgement that four staff received preservice training.

There is no acknowledgement that four staff received any orientation.

There was no proof of diploma on file for three out of four files checked.

First aid and CPR training was missing on file for one out of four staff files checked.

Date of employment was missing on file for three out of four staff files checked.

TV cord in one of the classrooms used by children was accessible to children.

Operation did not label cots with the child's name for naptime, however this was corrected when the director created a cot chat.

Date of admission was missing on file for two out of four records checked.

One child was observed after diaper change without washing her hands. This was corrected when caregiver washed the child hands.

Caregiver was observed changing diaper before naptime. However caregiver did not wash her hands after the diaper change.

Outdoor playground still has some hazards on the ground which needed to be removed.

Diaper changing mat has not been installed.

Mats or cots for napptime was not available during inspection.

There was no activity posted in each classroom that will be ued by children.
