Two infants ages 9 months old and 15 months old were observed being combined with the older children ages 3-4 years old during the inspection. This was corrected when the infants were brought back to their classroom during the inspection.

Three play yards mats located in the infants classroom were observed broken and did not fit snugly to the side of the play yards.

Garbage can was observed accessible to children in the restroom. This was corrected when the garbage can was moved away from the children

There was chipped paint on the walls, and stains on the ceiling observed in each room of the operation.

One play yard in the infant room did not have a tight fitting sheet. Note: Corrected at inspection when caregiver switched out the loose fitting sheet with a tight fitting sheet.

The feeding instructions for one infant age 11 months old was not available for review during the inspection.

It was observed that there was a broken piece of wooden fence causing a hole observed from the bottom of the fence in the playground

An 11 month old infant was observed sleeping in a crib with blanket during the inspection. NOTE: This was corrected, the blanket was removed from the crib by a caregiver.
